Job Type Full-timeDescriptionJob SummaryWorking under the direction of the Clinic Manager, the Clinical Nurse Supervisor will provide oversight to Minidoka Medical Center's assigned clinical staff. Duties include planning, coordinating, assigning, supervising and managing the work of professional and non-professional nursing personnel engaged in performing nursing services. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. May advise patients on health maintenance and disease prevention or provide case management. Assist surgeons during surgery by performing duties such as vital signs, medication reconciliation, or medical procedures. Perform preoperative and postoperative duties to facilitate patient care.Duties and Responsibilities Maintains an effective internal tracking system for identified patients. Coach patients/families toward successful self-management of their diagnosis. Utilize tools and documents that support a guided care process, collaborate with patient/family toward an effective plan of care. Assess patient and family's unmet health and social needs. Provide effective communications to improve health literacy. Develop a care plan based on mutual goals with the patient, provider, and family. Create ongoing processes for patients/families to determine and request the level of care coordination support they desire over time. Facilitate patient access to appropriate medical and specialty providers as well as other care coordination team support specialists (e.g., Diabetes Educator). Cultivate and support primary care and subspecialty co-management with timely communication, inquiry, follow-up, and integration of information into the care plan regarding transitions-in-care and referrals. In partnership with other clinic primary care provider nurses, serves as the contact-point, advocate, and informational resource for patient, family, care team, payers, and community resources. Ensure effective tracking of test results, medication management, and adherence to follow-up appointments. Facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed. Acknowledges patients' rights on confidentiality issues, maintains patient confidentiality at all times, and adheres to HIPAA guidelines and regulations. Cultivates effective partnerships, effectively collaborates with all practice providers (Physician, Nurse Practitioner, Physician Assistant and other licensed allied health team-members). Supports and maintains a culture of safety and quality. Medical and surgical asepsis is carried out during treatments and special procedures. Meets current documentation standards and policies. Assists in answering clinic telephone calls in a professional manner; relays patient information and concerns to attending physician. Ensures an adequate stock of supplies and proper functioning of equipment. Actively engage in quality improvement activities. Performs other duties as assigned within the RN scope of practice.RequirementsMinimum Requirements Associates Degree or higher in Nursing of an accredited school of nursing and holds current licensure in the state as a registered professional nurse One (1) year experience in the ambulatory, physician/medical clinic setting BLS Certification Must be able to pass any and all background checks required by lawSkills/Competencies Read, write, and comprehend instructions, short correspondence, and memos in English. Demonstrates a positive, respectful attitude and professional customer service. Demonstrates understanding in use of IT resources, Microsoft office, and patient databases. Demonstrates effective delegation skills to streamline operational workflows and optimize inter-office resources. Demonstrates the ability to be flexible, organized and function under stressful situations. Effectively present information in one-on-one and small group situations to customers, clients, and other employees of the organization. Demonstrates evidence of essential leadership, communication, education, collaboration, and counseling skills. Ability to identify and implement appropriate patient communication strategies and overcome accessibility barriers, as required. Proficient in communication technologies (email, cell phone, etc.). Effective organizational skills and demonstrates ability to maintain accurate notes and records. Add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Compute rate, ratio, and percent and to draw and interpret bar graphs.Work EnvironmentThe work environment described here are representative of those an employee encounters while performing the essential functions of this job.While performing the duties of this job, the employee frequently works near moving mechanical parts and is frequently exposed to fumes or airborne particles.
