Announcement #2025-013Issue Date:01-15-25Closing Date:01-28-25Clinical Care NavigatorBehavioral HealthDepartment of Tribal HealthHourly Wage: $20.10/Regular/Full-TimeThis individual will have direct experience with mental health services and will utilize those experiences to collaborate with counselors and clients to challenge the person to a continuum of coordinated mental health treatment and wellness services. The Care Navigator will be responsible for completing a whole-person needs assessment that determines the client's social determinants of health and for developing a guided care plan that addresses needs across life domains. This role requires familiarity of community services because of the need for managing communication and coordinating services between agencies necessary to help children, youth, and their families get the services they need.Examples of Work Performed:Assisting in clinical, emotional and support services as needed.Providing assistance in mental health and interdisciplinary treatments as directed by Clinical Supervisor.Coordinate with patients with Therapists, ANRP, and Psychiatrist for mental telehealth services.Provide client and community engagement services.Prevention, community education, and community organizing.Routine client contact, screening, assessment and evaluation.Complete data entry as assigned.Reviews for completeness of eligibility of documents such as client case files and Community resources and referrals.Maintain filing system, compile and enter information from a variety of sources into computer database(s), verify data, identify issues and research/correct as appropriate.Other duties as assigned by Clinical Supervisor and/or Program Director.Knowledge, Skills and Abilities: Ability to work professionally and respectfully with people from diverse backgrounds and cultures.Excellent customer service skills.Strong written and verbal communication skills, including over the phone.Experience handling confidential and/or sensitive information with attention to detail.Ability to work well under pressure, and maintain composure during stressful situations.Ability to work in a high volume, fast-paced call center environment, in which changes to policies may occur frequently.Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook) and other web-based applications.Ability to follow directions and be self-directed in time management.Ability to work positively in a team environment and independently.Experience providing crisis counseling and/or dealing with crisis situations in a sensitive manner is preferred.Minimum Requirements:Associates Degree Mental Health, Chemical Dependency, Social Work or related field.Three years of work related experience with direct service with clients/patients in a Social Work field.Required to pass pre-employment drug test.Required to pass pre-employment background check.Required to maintain confidentiality.Enrolled Yakama Preference, but all qualified applicants are encouraged to apply.
