Clinical Care Partner LPN - Rheumatology Clinic - Nyack Hospital : Job Details

The Clinical Care Partner (LPN) is responsible for providing exceptional nursing care to our patients at the Rheumatology Clinic within Montefiore Nyack Hospital located in Nyack, New York. Displays an understanding of the developmental needs of the patient when planning and implementing treatment. Displays competency in obtaining information, familiarity with medications, and processing of refill prescriptions. Works closely with the Physician and other members of the clinical team. May perform phlebotomy and/or injections for the patient and may call back patients with lab test results and diagnosis. The hours are Monday - Friday, 8:00am - 4:30pm or 8:30am - 5:00pm. No weekend coverage required. Associates degree preferred. Experience: 1-2 years' experience as an LPN within a Physician practice preferred. Training/Certification: BLS Certification Licensure: NYS LPN License, Current NYS Registration Other: Familiarity with using an electronic medical records system; EPIC preferred. Effective communication skills with patients and their family members. Physicians and other members of the multidisciplinary health care team.
