Agency Overview: The Safe Center LI, Inc. (TSCLI) is the leading provider of services to victims of interpersonal violence in Nassau County. The agency provides comprehensive services to victims and survivors of domestic violence, sexual assault, child abuse, human trafficking, and elder abuse. The Safe Center is a thought leader that seeks to develop and implement innovative interventions to meet the multifaceted needs of victims and survivors.Summary: The Safe Center currently functions from a no wrong door philosophy, where victims and non-offending family members are assessed comprehensively for concrete and service needs upon initial engagement with any program of the agency. As needs are identified, clients are connected to an Advocate or Case Manager based on their identified need. This results in a holistic wraparound response ensuring barriers to the effectiveness of clinical services are addressed early in treatment, helping optimize the effectiveness of services for the entire family system.This Clinical Case Manager will assist the Clinical Intake Coordinator with the front-end triage of cases, conducting phone screenings and assisting with referrals out, among other things. The Clinical Case Manager will be familiar with both grant funding and Article 31 OMH clinical guidance and standards of care. They will help enhance collaboration with referrals to psychiatric alternative levels of care.The Clinical Case Manager will help develop linkages to mental health higher levels of care such as Psychiatric hospital care or Outpatient Intensive Services. They serve as a bridge between both internal and external systems in which clients are involved, including criminal justice, family court, social services, and healthcare. They provide case coordination, maintain an overview of client case status and the progress of all involved systems/parties, and work to resolve system obstacles, minimize contradictory approaches, and support complimentary efforts to serve the clients' needs and support their healing. This position reports to the Director of Advocacy & Case Management. JOB DUTIES AND RESPONSIBILITIES:Provide comprehensive support to a caseload of adult DV, RSA, and Human TraffickingDevelop client-centered, strengths-based, and trauma-informed service plans that outline concrete actions toward meeting clients' needsOrient clients to systems processes and provide emotional support and accompaniment throughout the referral to alternative levels of careRisks and needs assessment, safety planning, and crisis interventionAssist clients with obtaining victim compensation, meeting concrete needs, and ensure successful linkages to medical, mental health, and other community-based services by maintaining strong partnerships with community partners Advocate on behalf of clients and coordinate case conferences with multidisciplinary partners, including medical personnel, law enforcement, prosecutors, family court, clinicians, and other community providersUtilize web-based electronic health record databases to maintain case notes and statistical recordsOther duties as assigned by the Senior Director of Clinical ServicesQUALIFICATIONS:Bachelor's degree in Social Work, Sociology, Psychology or related fieldTwo years' experience in the areas of case management/casework, crisis intervention, and/or referral coordination preferredExperience working with trauma survivors, knowledge of dynamics of interpersonal violence, and the ability to adopt a client-centered/strengths-based approach and growth mindset is preferredLeadership skills are strongly preferredSpoken and written fluency in Spanish is strongly preferredOne must be able to utilize the following technology: computer, fax machine, copy machine, Microsoft Office programs (Outlook, Teams, Word, Excel, PowerPoint, OneDrive, and SharePoint), and ZoomHOURS, COMPENSATION & BENEFITS:Position requires the use of a car and a flexible scheduleFull-time, non-exempt (35 hours/week)Monday through Friday 9AM-5PM (may need to flex time to meet program needs) Includes medical, dental, vision benefits and a generous paid time off packageAccess to National EAP services through TSCLI Ability to participate in self care activities provided by TSCLIMileage reimbursement provided for agency-related travelPosition may require overnight travel to conferences/trainingAnnual salary: $42,000 / $47,000 BilingualThe Safe Center LI is an equal opportunity employer.
