JOB PURPOSE: Serves as the clinical lead for new market / growth. Assumes initial responsibility for the management of internal providers. Collaborates with the Medical Director / Chief Medical Officer on managing the relationships with the external relationships with all community-based providers that are not directly owned or managed by CenterLight HealthCare System (CHLS). Collaborates with CenterLight partners, community-based organization. Facilitates the implementation of the clinical model in all its aspects to maximize the quality delivery of services to CLHS members and reduce costs system-wide. Collaborate with the senior leadership on growth and generally all new market implementation activities. JOB RESPONSIBILITIES: Providers Management Be able to articulate the vision and mission of the organization and the population health value based clinical framework. Support the recruitment and onboarding efforts of the new providers. Manage the performance of all nurse practitioners and physician assistants. Help establish high-quality relationships with non-CenterLight providers throughout the market. This includes PCPs and specialty physicians, group practices, hospitals, and ancillary providers that deliver care to the CenterLight participants. Support the providers and ensure their alignment with the larger clinical strategy of CenterLight. Participate in cost containment activities, including curb excess utilization of medical resources and pharmacy trends with external providers and institutions. Only act within the scope of the individuals authority to practice. Meet a standardized set of competencies for the specific position description established by the PACE organization before working independently. Acting member of the IDT. Quality and Clinical Operations Collaborates with the interdisciplinary team on all care planning requirement and be the steward of process improvement and effective communication across teams. Support case conference excellence by ensuring all PACE providers are prepared and receiving timely case notes and medical orders. Provide clinical input, when necessary, with internal or external stakeholders. Work with all providers, internal and external, to ensure accurate, timely, and complete implementation of any initiatives pertinent to care planning or to improving the overall quality care of CLHS members. Conduct policies review as necessary and collaborate with the senior leadership on medical and clinical policies development. Ensure Diagnostic and Testing Center is up to the highest standards from a regulatory perspective. Manage the resolution of practice-specific and network-wide policy issues as needed. Support other quality initiatives deemed necessary by the organization. Develop standard work and standardized operating procedures SOPs for the diagnostic and testing center DTC. Participates in safe care transitions initiatives and process improvement. Informatics and Clinical Model Implementation Lead the efforts for EMR optimization including needs assessment, process and workflows mapping and end user support. Partner with the IT department to prioritize the optimization initiatives and achieve desired results Provide support to the providers needs on a routine basis and as deemed necessary. Collaborate with the medical director on rolling out the foundational blocks of advanced primary care including empanelment, appropriate enrollment and risk stratification and tailored offerings based on population segmentation. Help implement a population health framework for managing the CenterLight population. Help the providers focus on identifying the high-risk membership, engaging them, determining their needs, and targeting them with the right interventions. Work with Care Delivery and the Quality Leadership to develop and track the effectiveness of plans to eliminate avoidable hospitalizations, and SNF admits. Participate in all clinical review activities, recommend and implement targeted clinical programs (palliative care, diabetes management, etc.). Develop workflows and SOPs as deemed necessary to facilitate the roll out of new clinical initiatives. Schedule: 8:00AM 5:00PM Weekly Hours: 40 QUALIFICATIONS: Education: Board certification as an NP. Experience: Minimum of three to five (3-5+) years of clinical experience, preferably in the managed care space, value based/ advanced primary care organization, digital health startups, or a PACE program. Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring. License: Active unrestricted medical license in the state of New York and new market state. Additional Requirements: Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function. Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact. Must have a Medicaid Provider ID and a Medicare Provider ID in good standing. Demonstrate a good understanding of managed care tactics, quality management, HEDIS, peer review, and utilization management (UM). Interest in building connections and relationships, skill in influencing, and consensus-building. Emotional intelligence, curiosity, and having a growth mindset. Ability to communicate effectively with multiple constituencies. A good listener. Physical Requirements Individuals must be able to sustain certain physical requirements essential to the job. This includes, but is not limited to: Standing Duration of up to 6 hours a day. Sitting/Stationary positions Sedentary position in duration of up to 6-8 hours a day for consecutive hours/periods. Lifting/Push/Pull Up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc. Bending/Squatting Must be able to safely bend or squat to care for patients, use medical supplies, etc. Stairs/Steps/Walking/Climbing Must be able to safely maneuver stairs, climb up/down, and walk to access work areas Position requires the individual to be able to travel, and walk between sites/locations and work areas throughout the day. Agility/Fine Motor Skills - Must demonstrate agility and fine motor skills to operate and activate equipment, devices, instruments, and tools (ie. typing, use of medical supplies, equipment, etc.) Sight/Visual Requirements Must be able to visually assess patients, read orders type/write documentation, etc. with accuracy. Audio Hearing and Motor Skills (language) Requirements Must be able to listen attentively and document information from patients, community members, providers, etc., and intake information through audio processing with accuracy. In addition, must be able to speak comfortably and clearly with language motor skills for customers to understand the individual. Cognitive Ability Must be able to demonstrate good decision-making, reasonableness, cognitive ability, rational processing, and analysis to satisfy essential functions of the job. Salary Range (Min-Max): $155,000.00 - $175,000.00
