Job Title: Clinical Doc Integrity TL Requisition Number: 39106 Employment Type: Full Time Division: HEALTH INFORMATION MANAGEMENT Compensation Type: Salaried Job Category: Business / Professional Hours Worked: Location: John Peter Smith Hospital Shift Worked: Job Description: Job Summary: The Clinical Documentation Integrity Team Lead will build on the Clinical Documentation Integrity (CDI) knowledge base and take a proactive role in growing accuracy, integrity and overall quality profile of the program. This position will take on special assignments and projects to improve the existing infrastructure in the department with respect to departmental education, data analytics, and quality outcomes. Under the direction of the CDI Manager, the incumbent will perform evaluation of patient safety indicators (PSIs), value-based purchasing (VBP) initiatives and mortality statistics, and implement performance improvements with CDS staff, coding department, and providers to improve quality and financial outcomes. This role will perform routine CDS chart audits of query compliance, quality CDS chart reviews and accurate DRG assignment, provide educational feedback to staff, and submit summary reports to CDS Manager. This role will function as an educator and an auditor. Essential Job Functions & Accountabilities:
  • Collaborates with leadership to assess the learning needs and competencies of the department and identify issues for education and training to address specialty specific documentation needs.
  • Designs, delivers and assesses learning activities and curricula related to competencies, departmental policies, regulatory requirements, and the use of information technologies.
  • Performs CDI performance audit assessments to ensure accurate and complete data capture for the CDI program. Presents recommendations and performance metrics to management/staff regularly regarding documentation opportunities and areas of improvement.
  • Identifies workflow improvements, performance/quality opportunities, and training topics.
  • Compiles, analyzes and evaluates collected audit data and reports to identify opportunities for process improvement and educational opportunities for CDI staff, coding staff, and physicians and other health care professionals.
  • Collaborates with coding staff and management during the CDI/Coding reconciliation process.
  • Assists with retrospective chart reviews regarding payer denials and appeals, RAC reviews, PEPPER reports, and other internal review needs.
  • Performs second level DRG reviews of medical records to increase the accuracy, clarity, and specificity of provider documentation.
  • Performs mortality reviews.
  • Facilitates peer review.
  • Develops recommendations as appropriate for documentation templates and physician query templates based on coding and regulatory changes, and data quality review findings.
  • Provides education, as needed, for new or existing staff, coders, physicians, and other health care professionals.
  • Functions as an expert resource for the CDI program.
  • Participates in the interviewing and hiring of new staff and administer new hire orientation. Acts as a preceptor and ensures follow up as needed.
  • Assist with IT strategy and implementation with Epic team and external software vendors. Creates joint standards for chart review and querying by diagnosis.
  • Reviews query type trends and report out on physician query response times. Monitors CDS audit trends and ensures compliant and effective queries.
  • Identifies areas where clinical documentation and coding can improve workflow. Develops policies/guidelines to assure a smooth and compliant CDI program.   • Qualifications: Required Qualifications:
    • Associates Degree in Nursing, Health Information Management, Health Informatics, or a related field of study from an accredited college or university.
    • 5 plus years of relevant acute care clinical experience.
    • 3 plus years of experience as a Clinical Documentation Improvement Specialist.
    • 1 plus year of experience in education development.
    • If applicable team members in this job that hold a professional licensure or credential must keep licensure and credentials current.
    • ACDIS administered certification of CCDS OR AHIMA administered certification of CDIP.
    Preferred Qualifications:
    • Bachelor's Degree in Nursing, Health Information Management, Health Informatics, or a related field of study from an accredited college or university.
    • 7 plus years of relevant acute care clinical experience.
    • 4 plus years of experience as a Clinical Documentation Improvement Specialist.
    • 3 plus year of experience in education development.
    • Current Medical Coding certification through AAPC, American Health Information Management Association (AHIMA) AND/OR through an organization accredited by the National Commission for Certifying Agencies (NCAA) for Medical Coding (e.g., Certified Professional Coder (CPC), Certified Outpatient Coding (COC), Certified Inpatient Coder (CIC), Certified Coding Associate (CCA), Certified Coding Specialist (CCS), or Certified Coding Specialist-Physician-based (CCS-P)).
