Job Description: Clinical Intake Manager
Summary and Objective
The clinical intake manager (CIM) is primarily responsible for facilitating timely admissions into CHS's SNF, IRF, home health (HH), and ALF lines of business through a consistent presence in primary referral sources and conversations with patients and their caregivers. The CIM role, upon referral, is to interface with the appropriate hospital/clinic provider and case management personnel making the referral to collect and analyze critical patient information and determine the most appropriate post-acute level of care for admission to CHS. The incumbent is the face of CHS to potential referrers, patients, and the community. As a leader at CHS, this role is responsible for developing a strong culture and effective communication among the sales and intake team to achieve budgeted occupancy rates for the assigned CHS region/campus in conjunction with the supervising sales manager and associated EDs.
Knowledge and Experience Requirements
- Active RN, APRN, PA, or PT license to practice in Florida or LPN with minimum of 10+ years experience; RN strongly preferred
- Three to five years of experience providing direct patient care in an acute or post-acute healthcare facility or agency
- At least two years of experience working in a care transitions, liaison, intake manager, or clinical sales role
- Working knowledge of admission criteria of patients receiving SNF, IRF, HH, and/or ALF services; including screening clinical information to facilitate post-acute services admission
- Knowledge of Medicare, Medicaid, managed care, Medicare Advantage plans, and other private insurance, effectively navigating the patient referral process across various payer categories
- Ability to function in a fast-paced environment; excellent customer service and communication skills
- Knowledge of computer office/clinical software and business intelligence reporting; proficiency in using healthcare software and electronical health record systems
- Current valid State of Florida driver's license
- Ability to read, write, and understand English; multilingual preferred
Essential Functions
- Coordinate all SNF, IRF, HH, and ALF admissions within the assigned region/campus market area from the time of referral to patient admission (arrival at facility or start of the care visit).
- Collaborate with hospital discharge planners, case managers, physicians, and other clinical providers to assess patients' post-acute care needs and facilitate admission to the appropriate CHS services.
- Recognize barriers to admission, and respond appropriately to mitigate them in a timely fashion.
- Review medical records, treatment plans, and patient history to determine and make recommendations for the most appropriate level of post-acute care needs.
- Discuss cases with physicians and other providers; clarify and document orders as needed for admission to CHS services.
- Communicate with families to provide information about post-acute care options, benefits, and potential outcomes; follow up consistently to close the sale and secure admission to a CHS facility/agency.
- Maintain a consistent presence in priority-identified referral sources as directed by the sales manager; establish critical relationships with physicians, hospital case managers, health system leaders, and other referral sources to drive business to CHS's SNF, IRF, HH, and ALF service lines.
- Accept referrals from all assigned customers, gather the essential documentation, and interact with the patient and their family in a “face of CHS” role.
- Communicate timely and completely all steps of referral process workflow, including collecting and entering data and information in the sales CRM and transferring required information to the CHS patient navigation center for seamless admissions.
- Keep the patient navigation center team appraised of changes in providers' requests, changes in patient status, and/or unusual circumstances requiring immediate follow-up prior to admission.
- Build relationships with all personnel who influence the referral process both internally at CHS and within the customer locations assigned.
- Educate physicians, case managers, and insurance companies on CHS's facility-/agency-specific programs.
- Attend in-person (when possible) UR Discharge planning meetings in assigned CHS facilities to facilitate referral capture to other levels of care (HH, ALF, SNF, etc.).
- Provide input to the sales manager, EDs, and director of business development into the region/campus sales plan; identify opportunities for process improvement or new market opportunities.
- Monthly reporting of KPIs and sales initiatives performance metrics to EDs and other key stakeholders for assigned region.
- Loyally represent CHS's brand to patients and the community.
