Assists the organization in acquisition and retention of customers. Responsible for supporting the patient through the continuum of care by the coordination of services, programs and personnel. Facilitates and ensures the achievement of goals with the use of appropriate resources and multidisciplinary collaboration to provide a coordinated experience for patients and families.

Position: Clinical Liaison HomeCare
Shift: Day shift plus 1 weekend per month; 24 hours per week
Experience:
1. RN candidates must have a minimum of three (3) years clinical experience. Home health or hospice experience preferred.
2. Minimum of one (1) year marketing/sales experience preferred.
Education:
Bachelor's degree in nursing required
Licensures/Certifications:
1. Current license to practice nursing in PA, OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
Requirements:
This position requires you to wear a respirator as part of the job requirement.
