Essential Functions:The Clinical Liaison is responsible for coordinating & generating hospice business in both existing and new accounts,identifying new markets and maintaining existing service as well as coordinating care for referred patients. The ClinicalLiaison's primary responsibility is serving as a liaison between the agency, hospitals, medical community and other referralsources. This position educates the community and the medical profession/referral sources regarding hospice services. Thisposition involves daily interaction with patients, medical professionals, other referral sources, and the community to assurecontinuity of care and to coordinate appropriate communication and documentation. This position works closely withagency personnel to ensure that patient care is handled efficiently and effectively. The Clinical Liaison is regularly engagedaway from the employee's place of business in performing these duties.
Qualifications:JOB QUALIFICATIONS
LICENSURE: Registered Nurse, Licensed Practical Nurse, or Respiratory Therapistcurrently licensed in the state. Must successfully perform and comply withall credentialing as required by hospital/healthcare facility
EXPERIENCE: 2 year experience in a clinical care setting or hospice preferred.
SKILLS: 2 year experience in a clinical care setting or hospice preferred.Requires proven interpersonal, coordination, and leadership skills with abilityto communicate effectively. Requires practical and theoretical knowledge ofhospice. Demonstrates active involvement in professional organizations andcommunity activities. Satisfies expectations and requirements of externalcustomers. Skilled in conflict resolution. Responds to suggestionconstructively.
