Job Location : New York,NY, USA
Job Summary: Working in an assigned territory, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families and working with case managers to ensure optimal transition from hospital or other facilities to home.
Minimum Qualifications
1. BSN preferred and current license to practice profession in the state where employee is providing service.
2. Current NYS driver's license and possession of a registered and insured motor vehicle.
3. Two years of experience working in an acute setting; one year of home care and/or hospice experience preferred.
4. Working knowledge of community resources
5. Ability to meet, collaborate and work efficiently with others in the community.
6. Knowledge of entitlement programs.
Additional Desirable Qualifications
1. Must be proficient in the use of Microsoft programs for internal and external communications, presentations, database tracking/reporting, and spreadsheet applications.
2. Must have excellent oral communication skills.
3. Is self-directed and possesses the ability to work with little supervision.
