Partners with patients and their primary physicians to develop customized care plans based on their individual needs and preferences. Collaborates with physicians, nurses, health professionals, social work, and others to ensure appropriate tests and treatments are delivered in a timely fashion. Advocates for the patient. Balances care needs and financial considerations to ensure efficient and effective treatments are achieved.Melanoma & Sarcoma Oncology Nurse NavigatorKey Responsibilities:* Deliver risk assessments and shared decision-making visits to patients who qualify for a low-dose lung screening CT scan.* Guide lung screening patients through their post-screening workups and diagnostics with compassion and expertise.* Timely Access: Ensure patients receive prompt access to vital diagnostic tests and treatments, facilitating a smooth care experience. Serve as a conduit and clinical connection between surgical oncology, medical oncology, radiation oncology and all ancillary care providers as needed in the program.* Collaborative Coordination: Work closely with our dedicated teams of nurses and physicians to develop and implement personalized treatment plans.* Innovative Contributions: Engage in screening and tumor board discussions, helping to shape innovative diagnostic and treatment strategies for complex cases.* Seamless Communication: Foster effective communication between nursing and physician teams to optimize patient care.Why This Role is Exciting:* Be at the forefront of cancer screenings, working with advanced technologies and diagnostic options.* Play a crucial role in enhancing patient outcomes and quality of life through effective coordination.* Gain valuable experience in a fast-paced, supportive environment while making a significant difference in patient lives.* Access to world class Precision Oncology Program and clinical trials* Requires an Associates of Nursing (ASN).* Bachelors of Nursing (BSN) preferred.* Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.* Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.* Requires Basic Life Support certification through the AHA.* Other advanced life support certifications may be required per unit/department specialty according to patient care policies.* Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.* Requires knowledge of personal computers using various types of word processing and database software.* Requires clinical work experience, medical terminology.* Requires understanding of medical record requirements, regulations and policies.
