Job Title: Clinical Nurse SupervisorLocation: New York, New YorkEmployment Type: Full-Time - ExemptDepartment: Integrated Care Overview: - The Nurse Supervisor is responsible for conducting thorough evaluations for prospective residents (both internal and external) as well as re-evaluations of current residents. This role involves assessing residents' needs across various domains, developing personalized care plans, and collaborating with interdisciplinary teams to ensure residents receive the highest quality of support. Key Responsibilities: -
  • Conduct comprehensive home evaluations for prospective residents and re-evaluations of current residents using the ICH approved evaluation tools, Yardi EHR, and NY DOH forms.
  • Conduct evaluations in settings other than homes as needed, such as skilled nursing facilities, hospitals, etc.
  • Conduct comprehensive evaluations of residents' physical health, cognitive abilities, social needs, and daily living skills.
  • In collaboration with ICT at the community, develop individualized service plans based on evaluation findings, ensuring that residents receive appropriate support and services.
  • Collaborate with healthcare professionals, residents, and their families to discuss evaluation results and service plans.
  • Observe and reevaluate residents regularly to ensure that their service plans remain appropriate and effective.
  • Document evaluation findings and service plans accurately and maintain resident records in compliance with regulatory standards.
  • Provide guidance and support to residents and their families regarding available services and resources.
  • Participate in ICT and multidisciplinary meetings to discuss resident care and evaluation outcomes.
  • Stay updated on best practices and regulatory requirements related to assisted living evaluations and care planning.
  • Review previous nursing evaluations/medical notes to determine necessary adjustments/updates in service plans.
  • Lead an active role in gathering clinical information from various healthcare setting, necessary in determining the level of care required by a prospective resident to ensure the community can provide the necessary support and services. Education/Experience/Licensure/Certification:
      • Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN), licensed to practice in the state of New York with either:
      • A baccalaureate degree and a minimum of two (2) years of full-time, or the equivalent of full-time, clinical experience in nursing, where at least one (1) year of which was in a home health care agency, hospital, skilled nursing facility, or other post-acute setting; or
      • A diploma or associates degree in nursing and at least four (4) years of recent full-time, or the equivalent of full-time, clinical experience in nursing, where at least one (1) year of which was in a home health care agency, hospital, skilled nursing facility, or other post-acute setting
        • Ability to travel around downstate New York, including the 5 boroughs, Long Island, Westchester and parts of New Jersey.
        • Experience in assisted living, home health or long-term care setting preferred
