Amae Health is defining the standard of care for people with severe mental illness.

We're on a mission to deliver radically different care for the 14 million Americans suffering with a severe mental illness (SMI). Amae Health is an integrated, outpatient care provider for patients with SMI, treating their psychiatric and physical conditions through in-person care delivery with wrap-around services. We're Hiring!

Amae Health is seeking an energetic and resourceful Community Liaison to join our Community Engagement Team. This role will be full time, requiring access to a vehicle and a home address in the Raleigh, NC or Los Angeles, CA metro areas.

About the Job

In this role as Community Liaison, you will build and maintain meaningful relationships with key community stakeholders, advocate for our innovative care model, and champion our mission to provide compassionate, comprehensive mental health services. You will be deeply embedded in the community, with frequent touchpoints with healthcare providers, social workers, community agencies, and patient advocates.

This dynamic role requires agility, adaptability, and the ability to juggle multiple priorities. As a key point of contact, you'll navigate a fast-paced environment, switching between tasks such as connecting with patients, coordinating complex care transitions with social workers, reaching out to hospital systems, visiting emergency rooms, responding to urgent referrals, and tracking detailed information to ensure smooth care transitions. Strong communication, organization, and the ability to manage competing demands are essential.

You should be comfortable working with a variety of stakeholders and having a considerable amount of autonomy in organizing and prioritizing outreach.

Key Responsibilities

Community Outreach Essential Functions - 80%

  • As an ambassador of Amae Health, you will conduct outreach to local and regional partners, including healthcare providers, hospitals, community agencies, correctional facilities, treatment centers, and other referral sources, to promote Amae's integrated care model and build sustainable relationships.
  • Actively manage and grow enrollment in targeted regions through systematic outreach, ensuring a steady pipeline of patient referrals.
  • Evaluate and track the effectiveness of outreach initiatives, pivoting as necessary to meet growth goals.
  • Identify leads for service expansion and develop strategies for assigned areas.
  • Coordinate rapid responses to urgent patient situations, collaborating with hospital systems and social workers to ensure timely care transitions.
  • Provide initial case consultations to patients and families through video, phone, or in-person visits, often in high-pressure, time-sensitive environments.
  • Establish and maintain relationships with community-based resources (e.g., NAMI), and advocate for Amae Health's services.
  • Work closely with clinical staff to ensure that referred patients are connected with the appropriate care and support.
  • Meet or exceed established enrollment targets.

Collaboration, Planning & Documentation Essential Functions - 20%

  • Consistently plan, track, and document activities.
  • Complete documentation in a timely manner.
  • Assume other duties in support of organizational needs.
  • Attend weekly meetings with state-based teams, enrollment leads, and others.
  • Create 6 to 12-month citywide plans in collaboration with clinical staff and leadership.

About You

  • Applicants must reside within the city limits of the location posted.
  • 3+ years of client or customer-facing experience (e.g. in-person consulting, cold calling, digital outreach, business development, etc.)
  • Proven track record of building a referral base and relationships, such as business development and/or sales, community outreach, or referral management for a hospital system.
  • Current driver's license, proof of acceptable automobile insurance coverage, and reliable transportation.
  • Excellent oral and written communication skills, including well-developed public speaking skills.
  • Thrives in a fast-paced, mission-driven environment.
  • General computer skills with a working knowledge of the internet and the ability to independently learn and use Google Suite, Slack and CRM software.
  • Drive to take initiative and work autonomously.
  • Demonstrates high ethical standards of behavior, with authentic compassion and care for those suffering from complex mental health conditions.
  • Willingness to work flexible hours, including evenings and weekends as required.

Nice to Have but Not Required

  • Previous knowledge of, and passion for, mental health and wellness.
  • Experience in hospital discharge planning, case management, or behavioral health.
  • Experience working within or directly with one or more of the large safety net hospitals in the region.
  • Direct experience interacting with disadvantaged populations.
  • Experience with digital outreach and advocacy.
