JOB DEFINITION:
The mission of the CN Guidance Youth ACT Team is to deliver comprehensive and effective services to individuals and their families who are diagnosed with severe emotional disturbance (SED) and whose needs have not been well met by more traditional service delivery approaches. The Youth ACT Team provides an integrated set of evidenced-based treatment andsupport services through a highly individualized approach. Services are provided in the community in places and situations where problems arise.
EDUCATIONAL REQUIREMENTS:
Bachelor's Degree in social work, psychology, rehabilitation counseling or related field required. Master's degree preferred.
EXPERIENCE REQUIRED:
One year experience working in a mental health setting at the Masters or higher level or two years' experience working in a mental health setting at the Bachelors level.
DUTIES AND RESPONSIBILITIES:
Assists clients with transportation and representation at court hearings.Completes referrals and linkage to community benefits, organizations and institutions that will assist the client in completing the stated treatment plan goals.Clinical support staff are responsible for working with the child and family in support of identified treatment goals through discrete and targeted service interventions, such as skill development or training and education. The clinical staff support the child and family in acquiring the necessary skills and abilities to manage their health, improve family relationships, and develop opportunities for prosocial activities and interactions.The clinical support staff who, in addition to performing routine clinical support team duties, also has lead responsibility for integrating educational and/or vocational goals and services with the tasks of all team members. This staff member provides needed assistance through all phases of the vocational service. Be able to assist team members with completing various assessments such as PHQ-9/a, suicidality assessments, relapse prevention and safety planning. Assist and coordinate in planning for community integration trips for Youth ACT RecipientsResponsible for Quality Improvement, Utilization Review and Performance Improvement activities including but not limited to data collection, analysis of outcome trends, and identification of service provision, which needs improvement, completion of case records and the timely submission of required documentations.Plan and coordinate all services, in the office or in the field, which support and supplement the rendering of case management and care coordination services to clients and their identified families including but not limited to applying for and maintenance of benefits through the Department of Social Services, Social Security Administration, housing applications etc.Must have an understanding of concrete services i.e., Medicaid, Medicare, DSS and SPOA housing. Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progressComplete QI Training during onboarding and participate in QI refresher training as needed.Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
