We are seeking a full-time (40 hours per week) Clinical Team Leader for our centers located in Maryland (cities include Baltimore, Fredrick, and the metro DC area). This position can be filled by a nurse practitioner or physician assistant. The hourly range for this position is $54 - $60 / hour. The Clinical Team Leader reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Clinical Team Leader collaborates on-site with the Center Leadership Team for center specific goals and priorities. The Clinical Team Leader collaborates with the Medical Director and/or Attending Physician for patient specific medical care related to the Sr CTL's clinical assignment. POSITION SUMMARY: The Clinical Team Leader provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The Clinical Team Leader works collaboratively with Center Leadership to achieve AlignMed Partners mission of improving the lives we touch through the delivery of high quality health care and everyday compassion. The Clinical Team Leader will split their time between clinical practice and team management, as determined by your manager (subject to change based on the needs of the AlignMed Partners team). •Clinical practice: The CTL will maintain a caseload for clinical APP services. In this role, the practitioner provides primary care services to nursing center residents in collaboration with the physician, and consultation and education to the center nursing staff regarding healthcare services •Team Management: The CTL is responsible for the clinical support of a defined APP team. The CTL mentors and supports APPs in their professional development, ensuring all clinical standards are met. They work in identifying, measuring, and evaluating individual and center-level outcomes within the oversight of the DCP/ACPM. •The Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Will consider other concentrations/specialty tracks based on experience and business need. The Physician Assistant must be a graduate from a program accredited by the ARC-PA and take the PA National Certifying Exam (PANCE) to use the PAC designation. PA state licensure required. NCCPA certification required. •A minimum of two years experience working within the adult or geriatric population is preferred. •In states where practice allows; APRN with an unrestricted professional license and DEA, for each state in which they practice. PA with an unrestricted professional license and DEA for each state in which they practice.
