Learn more at ProHealthCare.org Hours:Work 12 hours shifts, 3 nights per week, 7pm-7am, rotate weekdays and every 3rd weekend with 2 other peers. Holiday rotation. Work location will be Angels Grace Hospice \$2K sign-on bonus for C.N.A.\'s tied to 1-year commitment FTE: 0.90 What You Will Do: Responsible for answering all ProHealth Home Care & Home Hospice phone calls and triaging them accurately under the supervision of a registered nurse. This includes independently providing community resource information to callers. The role also entails project coordination related to regulatory annual requirements, audits and patient processes and completes intake functions while also providing CNA support to the staff and patients at AngelsGrace Hospice. What you will need: High school diploma or equivalent. Current WI CNA Licensure. Previous CNA experience preferred. Demonstrated experience and strong skills in customer service. Demonstrated ability to multi-task. Demonstrated ability to work independently and with little supervision. Passionate about providing excellent patient care and demonstrate compassion in end of life care.
