Under the direct supervision of the Director of Community Placement, perform all duties to assist in successful placement of people served. Work with employers to identify entry level jobs that match our job seeker's interests, skills, and needs. Develop hands on training opportunities to prepare job seekers for employment in specific fields/businesses. Will follow Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and other relevant regulatory guidelines.Essential Duties/ResponsibilitiesMission, Vision, & Values
To support thephilosophy through the agency's mission, vision, values, and strategic plan. Manage all responsibilities based on the agency mission, using individual choice and al appropriate policies, procedures, and regulations. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts. Assistance to the Director of Community PlacementAssist the Director of Community Placement in coordinating assessments, job development, job retention, follow up, and extended services for people served. Identify individual's support needs, assist in developing individual goals and objectives, and coordinating services. Process requests for supported employment services, including sharing information, working with other referral sources, scheduling work assessments, interviewing individuals and developing employment plans.Coordinate all procedures according to DHS/DRS requirements including placement plans, ongoing progress reports, meetings, staffings, monthly billing reports, and placement reports. Coordinate and network with community employers to develop appropriate employment matches. Assist in developing training materials including task analyses and instructional guides.Function as a team member, enlisting the cooperation of both persons served and staff in performing job duties.Perform other duties as assigned. Staff SupervisionProvide tours, orientation and introductions to new staff assigned to area of responsibility. In coordination with the Director of Community Placement provide training to staff assigned to area, including but not limited to; OJT (on the job training) and if needed CBTA (competency-based training assessment). Mentor new staff and existing staff to assure proper implementation of programs, personal assistance, and adherence to relevant policies and procedures.Implement coaching strategies (prompts, role model, support) when training staff assigned responsibilities. Program/Department Development and CoordinationImplement recommendations made by the Interdisciplinary Team in accordance with the Individualized Service Plans.Coordinate and network with community employers in order to develop appropriate employment matches. Shadow jobs targeted for job seekers, gather information, and assist Placement Coordinator on matching the appropriate job seekers to targeted jobs. Assure CQL recommendations are being implemented in the program area. Maintain assigned area in a safe, clean and orderly condition in an effort to provide an atmosphere conducive to instruction and training.Attend scheduled staff meetings and interdisciplinary team meetings as requested.Maintain an inventory of supplies and resources needed for work areas. Promote positive relationships with businesses in the community.Instruct, train, assess, aide, support and supervise individuals in assigned area; specifically, role model expectations and coach individuals for employment success at job sites.Assure staff are supporting individuals in the community and not doing the work for them. Develop and implement a training program to maximize the person's independence in all environments including assessments of that person's present skills and develop supports necessary for successful placement in community job. Utilize job/task analyses and/or behavior management techniques when applicable to train individuals on assigned tasks.As applicable, provide transportation to persons served.Provide natural learning opportunities, encouraging the person to be as independent as possible, at work and during recreation or leisure activities.Serve as positive role model for the person being supported.Participate in departmental or agency in-service training and continuing education programs to improve skills required of the position.Fiscal ManagementRecord production data and other data on progress through the use of Therap, agency data sheets, behavioral charts, and worker evaluation forms.Complete and turn in monthly progress notes for individuals in plan for DRS. Marginal Responsibilities Perform other duties as assigned.As a mandated reporter ensures all allegations abuse and neglect are reported as required. Education/Experience RequirementsBachelor's Degree preferred Supervisory experience neededOne year experience with persons with developmental disabilities. The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence. Ability to effectively present information in one-on one and small group situations to customers, clients, and other employees of the organization. Valid Driver's License or State ID.
