Program Description:

PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs - such as food, housing, employment, health insurance, and sexual health services - across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.

New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.

This is a grant-funded position ending March 31, 2027.

Position Summary:

We seek an experienced Community Engagement Manager to execute our WholeYouNYC SCN community outreach and engagement strategy in Brooklyn. The manager will work closely with SCN ecosystem partners (including Medicaid populations) to develop and implement strategies that ensure meaningful and culturally relevant integration of our SCN at the hyperlocal level. The Community Engagement Manager is critical to ensuring the WholeYouNYC SCN remains connected to the communities we serve and is responsive to its needs.

Specifically, the Community Engagement Manager will:

* Plan, organize, direct and manage all activities of the WholeYouNYC SCN community outreach engagement strategy within Brooklyn.

* Identify priority neighborhoods to deploy our community outreach and engagement strategy.

* Develop and implement tailored, on the ground outreach and engagement strategies and campaigns that raise community awareness about WholeYouNYC and promote partner engagement in the SCN.

* Organize and facilitate community events and activities that serve as a platform for dialogue and learning about WholeYouNYC including but not limited to community presentations, focus groups, surveying and listening sessions.

* Deploy our local community engagement CHW teams across Brooklyn to participate in community events such as health fairs, workshops, food pantries, and church events, providing direct support for SCN screenings and navigation, as needed.

* Serve as an SCN representative at community meetings, civic and neighborhood events.

* Identify and foster effective trust-based relationships with SCN partners, local organizations, community leaders and other key stakeholders that build and maintain meaningful relationships and connections between the SCN and the community.

* Plan community level sessions and workgroups with diverse stakeholders to understand population needs and the barriers preventing effective care coordination across the health and human services.

* Plan and implement mechanisms (e.g., town halls, surveys) for continuous feedback and engagement from CBOs, providers, community members and other SCN stakeholders.

* Assist Communications, Health Policy and other SCN network management teams with message development and the preparation of media, presentations, reports, newsletters and other SCN informational and marketing materials.

* Collaborate with network CBOs and other ecosystem partners to develop targeted messages that enable the SCN to reach broader communities and more Medicaid members.

* Proactively identify opportunities to help the network address unmet local population needs and share challenges, best practices and strategies to more effectively address their needs.

* Identify and assess barriers to WholeYouNYC participation for local CBOs and develop strategies to promote network engagement.

* Proactively stay abreast of local community needs, demographics, social drivers of health and outcomes to inform SCN engagement strategies and priorities.

* Monitor and evaluate the effectiveness of community engagement initiatives. Develop and implement remediation plans to issues and gaps.

* Directly supervise a team of Community Engagement Specialists (CHWs) providing day-to-day management and oversight of team activities, leadership, coaching and career development.

* Track, manage and report on the status of community outreach and engagement efforts to internal and external stakeholders.

* Perform other related duties, as assigned.

Qualifications and Experience:

* Bachelor's Degree or equivalent preferably in public health, health promotion or a related field.

* Minimum 3 years of progressive professional experience in community engagement and advocacy.

* Proven experience managing community projects and organizing events.

* Proven ability to build relationships and create partnerships that enhance community engagement.

* Prior work advancing health equity and addressing disparities in vulnerable and underserved New York City communities.

* Understanding of the health and human services sector in NYC/NYS and related government agencies.

* Knowledge of community engagement techniques in vulnerable communities.

* Ability to maintain relationships with individuals at various levels of the community from elected officials to Medicaid members residing in the community.

* Excellent organizational and interpersonal skills (verbal, written and presentation).

* Well-developed public speaking skills.

* Ability to demonstrate sensitivity and tact in interpersonal relations.

* Foundational knowledge of the social drivers of health (SDoH) and their impact on health outcomes.

* Possess a passion for community engagement.

* Project management skills.

* Ability to travel up to 5 days week (Mon - Fri) within local communities across Brooklyn.

* Willingness to work flexible hours, including evenings and weekends, as required.

* Salary : $77,000 - $77,000

Desired Skills:

* Bilingual language skills are highly desirable.

* Existing relationships and representation at the community level in Brooklyn (e.g., active participation on a local community board).
