Overview UF Health St. Johns Care Connect operates an information network, the Care Connect Information Network (CCIN), designed to connect clients with community resources and address social determinants of health through a secure, coordinated system. The Community Health Improvement System Coordinator plays a key role in troubleshooting and implementing solutions, driving the ongoing development of system software and managing data architecture. Focused on solutions-oriented support, the Coordinator ensures seamless system implementation across agency partners, develops training materials, and supports product use across the CCIN. Additionally, the Coordinator manages reporting to track outcomes, identify service gaps, and provide leadership with actionable insights through data analytics and solution-driven visualization. Responsibilities Develops and maintains data architecture. Assists software vendors in program development by providing feedback on design, implementation strategy, testing, and deployment. Leads system support meetings and office hours. Develops system-wide security training, best practice workflow, and project workflow in collaboration with program leads. Provides updates on system changes to impacted users, ensuring they are informed and trained on any modifications or enhancements. Designs and delivers streamlined, consistent training, creates training materials, and offers ongoing training support. Submits support tickets and troubleshoots electronic referral system concerns, working closely with system support staff to ensure timely fixes. Maintains all necessary paperwork for agency and user access to shared software. Acts as the liaison between Community Health Improvement staff, system users, and electronic referral system support staff, ensuring clear communication and issue resolution. Develops and implements data security management policies and procedures for privacy, confidentiality, appropriate access, and ethical framework for CCIN. Develops custom reporting to meet the needs of Care Connect leadership and partner agencies. Develops and reports network outcome measurements. Conducts system-wide audits and gap analysis. Possesses an in-depth knowledge of Care Connect program operations and the impact of their workflows on data management. Maintains and updates UF Health St. Johns Care Connect websites and forms embedded in the platform. Provides decision support to leadership through data analytics and visualization of potential solutions. Perform all other duties as assigned by management within job scope. Qualifications Education / Training Bachelor's Degree Related Area of Study Experience Requirements 1-year Database Management Additional Information: Level 2 DCF Background Check reequired upon hire
