Community Health Service Worker - Healthcare Support : Job Details

Community Health Service WorkerHealthCare Support is actively seeking a Community Health Service Worker to fill an opening with a Managed Care Organization in Macon, GA.Daily Responsibilities for Community Health Service Worker:

• Training will be in Atlanta for first week and client will reimburse travel, room, and board
• Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
• Connecting with community partners and/or organizations to build partnerships that help to address the needs of our members.
• Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
• 80% of the role will be remote
• This will be Contract to Permanent role, schedule is Monday-Thursday, 9am-6pm and Friday, 8:30am - 5:30pm (may have some OT and weekends, need to be flexible)

Required Qualifications for Community Health Service Worker:

• High school diploma or GED required
• Reliable transportation, GA driver's license, proof of insurance, clear DMV report
• Strongly prefer Community Health Worker Training/Certification
• 2+ years of community health, social work, social services, community advocacy, community outreach, member services, or member education experience
• Service coordination experience, need knowledge of community resources and service coordination experience
• Experience doing home visits
• Ability to WFH as needed

Preferred Qualifications for Community Health Service Worker:

• Bachelor's Degree
• Bilingual in English/Spanish
• MCO experience
• Prefer event experience (either someone who has planned/hosted community events, or someone who has represented their organization at an event)
