Medix is now hiring HYBRID Community Health Workers to service Fresno County!

Shift: Mon-Fri (8am-5pm)

Type: Hybrid (work from home/out in the field)

Requirements:

• Community Health Worker Experience
• Personal Vehicle, Valid Driver's License and Auto Insurance, this role does require driving (mileage reimbursement provided)
• Housing resource experience
• Familiarity with Cal-AIM

Duties:

• Perform consistent and frequent outreach services in assigned hospitals/healthcare systems.
• Build relationships with discharge planning staff.
• Establish and maintain communication procedures to accept hospital/healthcare system referrals for homeless patients.
• Contact homeless individuals prior to discharge, introducing available resources and the Outreach Specialist's role.
• Enroll patients in support programs and collaborate with existing providers to avoid duplication of services.
• Assist in developing and presenting reports for community stakeholders quarterly.
• Provide information, referrals, linkages, and advocacy to help clients access services and resources.
• Offer crisis intervention services for individuals experiencing homelessness.
• Assist clients in obtaining necessary documents and services (e.g., identification card, birth certificate, social security income).
• Identify appropriate temporary and permanent housing options for clients.
• Assist with housing applications and paperwork for supportive and subsidized housing.
• Survey the rental market for affordable housing and advocate for clients with prospective landlords.
