**Job Summary**

+ **The Community Health Workers (CHWs) are frontline public health workers who have a close understanding of the community they serve.**

+ **This trusting relationship enables them to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.**

+ **This individual would work closely with medical providers, primary care teams, and social service agencies to provide care coordination and connections to improve their health and general well-being through education and provision.**

+ **This individual would work in both clinical and community-based settings.**

**Education and/or Experience**

+ **High School Diploma or Equivalent required**

+ A minimum of two years direct experience preferred.

+ Experience with navigating local medical and social support systems preferred.

+ Possible bilingual and medical interpreter qualifications preferred based on specific position needs.

+ Possible knowledge and/or experience with certain medical diagnoses preferred based on specific position needs.

+ Must be from the community that is being served or have a familiarity of the community.

**License/Certification/Registration**

+ This position requires a valid drivers license and that, the employee is insurable by the Avera Health automobile liability insurance carrier.

+ Must complete the South Dakota Community Health Care Worker (CHW) Certification within 2 years of hire and maintain certification.

**Work Schedule**

+ Day Shift: Monday-Friday; 80 hours per 2 weeks

+ Occasional weekends and evenings for special events or speaking engagements.

**Pay**

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

+ $19.25 - $24.25 per hour
