Description Job Schedule: Per Diem Standard Hours: 1 Job Shift: Shift 1 Shift Details: This position is budgeted for 10-12 hrs/wk for 12 month grant with FLEXIBLE HOURS *This is a grant-funded position budgeted for 9-12 months of service. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.Project Overview:For this project, incumbent will be responsible for work related to two grants. There will be a focus on Breast Health, encouraging community members to receive screening mammography and promoting breast health. In addition, incumbent will focus on identifying health-related social needs and work to meet those needs through available resources and community connections. The incumbent will serve as part of the Mission Services/Community Health team and will be involved in many community health related programs.The community health worker will:Connect patients/community members and families to resources and services that address their social influencers of health.Provide referrals and connection to service.Participate in bi-weekly food distribution.Attend health fairs and other community-based health promotion events.Participate in Medical Mission program.Assist with CHW caseload as needed to ensure continuity of care.Ongoing data entry tracking client progress and interactions.If your resumedoes not include your CHW experience – which may be paid or unpaid – please make sure to either add the experience to your resume OR include a cover letter that outlines your experience.Qualifications Education:High School diploma or equivalent.Licensure:In process of working towards the CT Community Health Worker certification required.CT CHW Certification preferredExperience:100 hours of CHW worker experience preferred but not required.We take great care of careers.With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
