The Community Health Worker works closely with the care team and is responsible for providing assistance to UnityPoint patients that are being cared for in the Emergency Room space. Assists patients in identifying and accessing community-based resources. Provide resource education, referral, and follow-up to necessary resources to ensure optimal health outcomes.

Responsibilities
Care Coordination
* Promotes positive customer relations in dealing with patients, visitors, and other employees.
* Interacts with clients to promote a full range of health services; determines the benefit of each health program after assessing clients' willingness to enroll in available programs.
* Provide cultural and linguistic health education information and current resource materials to individuals and families.
* Utilize a holistic approach (considering physical, psych/social, spiritual, educational, safety and related criteria) appropriate to the age of the clients served in the assigned service area.
* Refers families to clinical, social, and financial support using an established algorithm to assess patient and family needs.
* Assist patients with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified community based resources.
* Advocate for individual and community health.
* Enhance patient's ability to effectively communicate with healthcare providers.
* Works with staff and providers to create a care plan and communicates with them regarding the status of referrals and outreach efforts.
Administrative Duties
* Maintain documentation of patients served and support provided.
* Complete data entry to track patient outcomes, identify challenges and recognize success.
* Maintain up to date information about community based resources in order to make appropriate referrals.
* Identifies trends in barriers experienced by families when seeking care and provides data to team members.
Qualifications
Education:
* High school diploma or equivalent. Graduated from accredited college or university in the field of Human Services, Psychology, Sociology or Social Work preferred.
Experience:
* Experience with mental health symptoms, diagnosis and behaviors.
* Strong understanding of community-based resources.
Preferred, but not required:
* Experience as a direct care worker
* Familiar with Community Resource
* Working in a fast-paced environment
License(s)/Certification(s):
* Valid driver's license when driving any vehicle for work-related reasons.
* Mandatory Reporter certification
* CPR Certified
Knowledge/Skills/Abilities:
* Writes, reads, comprehends and speaks fluent English
