Community Health Worker Field SupervisorBilingual (Travel Required)VillageCare is looking for an enthusiastic and passionate Community Health Worker Field Supervisor. The Community Health Worker Field Supervisor will oversee and lead the Community Health Worker Team by managing field staff and compliance regulations for the department. The supervisor will conduct field visits with the Community Health Workers to ensure visits are being conducted appropriately and within the conduct of the organization. The supervisor will manage schedules as appropriate including support the CHW Manager with key performance indicators for the department. The CHW Field Supervisor will also support the safety of the CHW Team during onsite visits.Essential Job Functions:Supervise and manage the activities of the Community Health Worker team, ensuring communication, scheduling and coordination across the Care Management team.Responsible for ensuring all CHW visits are completed in a timely manner.Conduct chart audits to ensure documentation in the electronic record is appropriate.Conduct field visits and supervision with the community health workers.Identifies training needs and conducts new hire orientation and ongoing refresher training for Community Health Workers Department, including training individuals in otherProvides manager with updates/concerns regarding the operations.Effectively manages escalations within the department by ensuring appropriate accountability, sense of urgency, communication and follow through to closureAssist with coordination, implementation, execution of projects within the CHW departmentPerforms and assists with staffing initiativesWe would like to speak to those who have 3 years of supervisory experience, preferably in a healthcare setting. Associates Degree required - Bachelors Degree preferred. Strong ability to solve problems independently and interact with an integrated team. Exceptional skills of organization, communication, professional interaction, human relation skills, and analytical skills required. This position covers all boroughs - 100% field work.
