CareStar, Inc. is currently seeking a Community Health Worker in the Clinical department to conduct program assessments using program andCareStar identified tools and processes. The Community Health Worker is a home-based position that works independently to manage a caseload as needed.Position DetailsFlex staff are employed on an as needed basis. When services are required by CareStar, Inc., the employee may work up to a maximum of 29 hours per week. Employees in this category are not eligible for benefits unless otherwise set forth in the Employee Benefit Eligibility Policy, but are eligible for mileage reimbursement pursuant to CareStar policy.Responsibilities Active license as RN, LSW, or LISW is preferred.12 months experience in home and community-based services within the past 5 years.Knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs is desirable.Adheres to the CareStar Rule in performance of job responsibilities.Understands and complies with CareStar Policies and Procedures.Maintains confidentiality related to patient information. Any unlawful disclosures of confidential information will be treated as a serious disciplinary offense.Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
