This individual participates in community and educational activities to support the mission and goals of the Healthy Women Healthy Families Project.Requirements* Participates in community meetings, events, and health fairs linking high-risk families to Central Intake for further referral. * Works closely with Referral Specialists. * Provides health education to teach individuals how to make good health decisions, incorporate healthy habits/activities, and avoid harmful behaviors. * Promotes health insurance/Medicaid enrollment. * Provides case management to women who are not eligible for referral to home visiting programs and are in need of support and services. * Networks with community partners in public health clinics, schools, colleges, hospitals, community centers, nonprofit organizations, and government agencies to increase referrals into Central Intake; assists with dissemination of HWHF materials in community through daily outreach. * Ensures confidentiality of all client information. * Participates in ongoing training sessions, conferences, and seminars. * Completes all documentation in SPECT system to collect data, and track daily activities and event contacts. * Attends and participates in appropriate Project meetings and activities, and other meetings as needed. * Performs other duties as assigned.Knowledge, Skills, and Abilities to Perform Essential Functions:+ Minimum requirement of high school diploma with two years of experience in MCH, other health or social related field, and/or Associate's Degree+ Valid driver's license, current insurance policy and car+ Knowledge of community resources+ Effective oral communication skills+ Ability to keep accurate records+ Ability to work well with a variety of people and in a team fashion+ Knowledge of communities they are serving preferred+ Bilingual English/Spanish preferredPhysical Requirements:+ Must be able to see to read documents and hear well enough to communicate with others+ Must be able to write, type on a computer, and use a telephone+ Must be able to operate a motor vehicle+ Must be able to lift up to 20 pounds+ Additional requirements to be determined with input from Director and HRReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Work Environment:Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions.
