Overview Provide culturally appropriate health education and information, mediation between communities and the health and human services systems, ensuring people get the services they need and providing informal counseling and social support, while ensuring continuity of individuals in UHA supported programs. (20%) Administer surveys and interviews at predetermined collection points, review assignments/surveys for completion and quality with updates to supervisors and other organizational and program leadership with required reports of UHA program initiatives. (40%) Participate in the development and implementation of a comprehensive, culturally sensitive, social media strategy that is informative and appealing to increase our web traffic, brand awareness, and audience participation. (20%) Work closely with supervisor(s) to develop daily/weekly plans and to evaluate on-going responsibilities, confirm daily/weekly objectives, and to effectively execute the work of the program to fulfill Urban Health Alliance objectives. (10%) Flexible schedules to accommodate for participants' availability and may include working evenings and weekends to support UHA initiatives through surveys, projects, and tasks as assigned. (5%) Maintain the highest level of confidentiality of program information including but not limited to: participants' identities and personal information at all times while treating all participants, regardless of their role or position, with respect and dignity at all times Qualifications All other duties as assigned by supervisor. Maintain a log, document, and track assigned caseload. Participate in at least one (1) authorized CHW training each month. Required Education High School Diploma or GED Required Licensure/Certifications - Basic Life Support (BLS) certification from the American Heart Association or the American Red Cross - Community Health Worker Certification (CHCW) from the Florida Certification Board within one (1) year of hire or transfer into the position. - Level II Background clearance is required. - Valid Florida or Georgia Driver's license with clean record, or violations not exceeding maximum points as provided in hospital policy. Preferred Licensure/Certifications Certified Community Health Worker (CCHW) from The Florida Certification Board Required Experience Less than 1 year healthcare related experience Preferred Experience N/A Necessary Skills 1. Strong written and verbal communication skills 2. Advanced Beginner in Microsoft Word, Excel, and Outlook Age of Patients Served Adult (18-64 years)
