Title: Community Health Worker Location: Upper East Side Org Unit: Finance and Research Accounting Work Days: Weekly Hours: 35.00 Exemption Status: Non-ExemptSalary Range: $25.44 - $28.57 *As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices Position SummaryThe Community Health Worker will work closely with both the Department of Obstetrics & Gynecology as well as community partner Life of Hope to implement program goals. The goal of the Health Immigrant Families initiative is to provide perinatal support services for communities that have unique cultural and language barriers to care and services. The CHW will plan an integral role in engaging and enrolling community members, providing educational counseling and a primary linkage between Downstate and community partners.Job ResponsibilitiesPatient engagement to ensure that practice patients are educated and aware of servicesSupport practice staff in implementation of the programPerform screening and assessmentLinkage to community partner including, supporting program activities delivered both on-site and at community partner.Meet regularly with community partners to ensure closed-loop referralsAssist in maintaining files and databaseWhere appropriate, outreach to educate and inform of programmingPerforms other duties as assigned by SupervisorEducationHigh school diploma or GEDExperience Minimum 2 years of applicable experience and/or equivalent experience. 2 years experience as a community health worker or similar capacity preferred. Oral proficiency in Haitian Creole is required.Knowledge, Skills and AbilitiesProficient in Microsoft Office Suite.Excellent communicator with a willingness to learn and collaborate with othersStrong work ethic.Comfortable working in multi disciplinary teams.Licenses and CertificationsPlus to have CHW or Doula experience.Working Conditions/Physical Demands Ability to travel to other sites; local travel.
