What You'll Be DoingYou will provide support to staff and families in a home-based setting, supporting families who have members coping with physical, emotional, socioeconomic, and/or social difficulties. The role includes community outreach activities, helping families practice skills learned during treatment, connecting families with resources, and assisting staff with administrative tasks. You will work closely with the Clinical Care Coordinators and Clinicians on the Family Stabilization Program team.Your Main PrioritiesWhile additional responsibilities are likely to arise on the job, you will primarily:• Complete a community health worker certification course as needed.• Schedule and attend in-home check-in sessions with families to support their treatment; help them practice skills related to reaching their identified goals.• Collect, assemble, and help families access community resource information.• Write progress notes in the Electronic Health Record (EHR) system in accordance with expected requirements and due dates.• Provide community support; identify and attend community events and engage in community outreach.• Attend required meetings to discuss cases, openings, progress, etc.• Accompany team members on in-home visits to support in the delivery of services as needed.• Transport clients to sessions as needed.• Provide support and guidance to individuals and families managing crises, and be part of an on-call crisis support phone rotation.Who You AreRequirements, Skills & Abilities:• High school diploma or equivalent required.• Experience working in child welfare, parent education, or home-based services required.• Knowledge of child and adolescent development, family dynamics, substance abuse, and/or crisis intervention preferred.• Bilingual in Spanish preferred.• Strong organizational, critical thinking and time-management skills.• Basic computer skills required; knowledge of Microsoft Office 365.• Ability to communicate and effectively interact with a diverse population.• Collaboration and teamwork.• Must have a valid driver's license, proof of auto insurance and car registration.• Must authorize Child & Family to complete a motor vehicle records check.• The ideal candidate will be required to meet all background checks and medical clearances required for the position.Physical RequirementsDirect Service Staff (Travel as part of position):This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs.
