As a Community Health Worker, you will play a crucial role in promoting health and wellness, engaging with individuals and families to provide vital education, resources, and support. Your efforts will empower community members to navigate the healthcare system and access essential services.In this dynamic position, you'll perform home visits, offering personalized health and social service navigation assistance through a vast network of community agencies and in-house clinical services.Your impact will be two-fold: first, you'll lead community-based activities that support an interprofessional service curriculum, teaching Roseman medical students to identify and address the social, economic, and cultural drivers of health disparities. Second, you'll collaborate with a CHW partner and healthcare providers to develop and implement tailored care plans for low-income, medically underserved households, enhancing health and social outcomes for everyone in the household.If you're ready to drive positive change and improve the health of our community, we want you on our team!Essential Functions:
  • Identifies the social and medical issues of enrolled household members; provides and documents needed referrals and follow-up.
  • Assess the needs of individuals and families to identify barriers to access and areas of support.
  • Gather and document relevant information to tailor services to community needs in our Wellup platform.
  • Motivates households to stay in the program by maintaining consistent communication with assigned members.
  • Travels between the Summerlin campus and the communities to implement the program's goals and objectives.
  • Assesses eligibility of households through in-person or tele-visits
  • Documents all outreach household interactions and attempted contacts in the electronic record system to communicate with faculty and student teams.
  • Supports the reporting for grants and contracts.
  • Directs telephonic and electronic communications related to outreach activities.
  • Schedules household visits and maintains outreach calendar.
  • Assists with the implementation of strategies aimed at recruiting households for GENESIS.
  • Provides updates on household visits, community events, and outreach activities.
  • Provide health education and coaching on topics such as nutrition, exercise, chronic disease management, and preventive care.
  • Assist individuals in navigating the healthcare system, including scheduling appointments, completing paperwork, and understanding medical instructions.
  • Collaborate with healthcare providers to advocate for patients and ensure their needs are met.
  • Organize and facilitate support groups, workshops, and community events to address specific health issues and promote healthy behaviors.
Qualifications:
  • A high school diploma or equivalent.
  • A Community Health Worker certification required.
  • Previous experience in community outreach, healthcare, social services, or related field preferred.
  • Experience working in low income, medically underserved and/or high-risk communities.
