Our goal is to be the most valued partner to health plans, service providers, and the community we collectively serve. How You'll Make a Difference: Arkos Health is seeking a Community Health Worker. The Community Health Worker will be responsible for providing ongoing support and expertise throughout assessment and planning of individual members' needs. This role is patient facing and similar to a community health worker. The overall goal of the position is to enhance the quality of patient management and to promote continuity of care and cost effectiveness through therapeutic interaction, supportive education, active listening, advocacy, collaboration with providers and community referral. Essential Duties and Responsibilities:
  • Provide support services and therapeutic interaction to plan members to promote member engagement in company programs
  • Provide services such as: community referrals, supportive services, crisis intervention, member advocacy in support of member and overall goal
  • Conduct member outreach through home visits and welfare checks to assess member for program participation
  • Work collaboratively with other agencies involved in member's care
  • Demonstrated ability to remain calm in crisis situations
Qualifications and Skills:
  • Must have current BLS/CPR certification
  • Must have proof of current TB test- company provided
  • Current Hep B test- or waiver - company provided
  • Current Flu shot- or waiver and mask
  • Experience interacting with people with behavioral health issues
  • Must have valid driver license and appropriate liability insurance
  • Must be comfortable driving in the field up to 80% of the time
