Community Health Worker

Health Center

General Responsibilities: Full-time 40 hours week. The community health worker (CHW) is a key member of the multi-disciplinary team within the primary care setting. The CHW screens patients for Social Determinants of Health (SDOH), engages patients, identifies needs, works to resolve barriers to care and ensures that referrals are made to appropriate community resources.

Specific/Significant Job Responsibilities:

* Screens for SDOH needs of patients in the primary care setting, assesses needs and provides intervention and referral to community resources.

* Maintains extensive knowledge of community-based organizations, resource availability and eligibility guidelines.

* Collaborates with community providers to establish connections with resources to improve patient health outcomes.

* Works with patients with chronic health conditions or behavioral health needs while assisting to ensure access into appropriate programs.

* Coordinates transportation scheduling and other logistics for eligible patients.

* Assists patients with applying for health insurance and understanding available insurance options.

* Works collaboratively with the interdisciplinary team to improve patient care and health outcomes.

* Remains available to provide immediate assistance/warm hand off's for patients presenting in need of resources during primary care appointments.

* On a monthly basis, tracks completed SDOH screenings, identified needs and referrals provided.

* Attends ongoing training for community health workers.

* Participates in continuing education programs, including one (1) annual training program on compliance with OSHA's Blood borne Pathogen Standard.

* Performs other duties as requested.

Required Qualifications: Bachelor's Degree in community health, social work, psychology or related field preferred. Community Health Worker (CHW) certification in Rhode Island required. Commitment to equity and reducing disparities based on race, ethnicity, immigration status, gender, sexual orientation/gender identity, and social economics. Desire to understand the concepts of institutional and structural racism and bias and their impact on the health of underserved communities. Experience serving a population of diverse educations, economic and cultural backgrounds. Experience with case management preferred. CPR Certification. Bilingual is an asset, but is not required.

Special Qualifications: Strong interpersonal skills as well as a collaborative approach and style of communication are needed to interact with patients of varied age, ethnicity and social backgrounds. Experience with motivational interviewing advising/counseling patients in health education activities. Experience working with an electronic health record (Next Gen preferred). Insurance Navigator certification and/or willing to obtain preferred.
