**15850BR****Title:**Community Health Worker**Department/Cost Center:**771 - Social Work Service**Job Description:**The Outreach Worker will provide direct organizational support to the Observation Unit and Emergency Department with a focus on expediting patients and arranging transportation. They will provide tracking and outreach services to ED patients who are identified as uninsured, no/low utilization of primary care to encourage linkage with a primary care provider, enroll in health insurance, support patient in understanding and self-management of personal health conditions, improve communication amongst providers, and assist with accessing appropriate care.**Salary Range:**$21.30- $28.28 an hour_The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations._**Auto req ID:**15850BR**Job Requirements:**1. Education: Bachelor of Social Work2. Experience: Service - related field work experience preferred3. Certifications/Licenses: valid NYS driver's license4. Skills: Developed interpersonal skills as well as verbal and written, communication skills, capacity to use Microsoft Office word and Excel data bases, strong organizational skills, capacity to operate with significant independence, self- motivation, empathy, acceptance of and interest in a diverse client population, ability to respond supportively and with persistence to the client families and show understanding of the community, and ability to maintain project records. The ability to work well and flexibly with a variety of individuals as needed. Willingness to work within the community, complete home visits and directly transport clients is further required.
