What You Should Know About the Community Health Worker Role:

* This is a Full-Time position, Monday-Friday, 8am-5pm

The Essential Duties Are:

Works with patients, providers, and multidisciplinary team members to assess, facilitate, plan, and coordinate an integrated delivery of care from acute care across the continuum. Responsible for building relationships with patients within their community settings. Be a Liaison between patient and our organization. Instruct and assist on programs within organization. Assists with non-clinical aspects of the Plan of Care with oversight by the clinical team. Interacts with all staff members, volunteers, and members of the medical community, as it relates to the fulfillment of the mission and goals of Pure Healthcare.

Qualifications:

* Associate degree in medical related field preferred (Tech, Community Outreach, Community Health Worker, Medical Assistant, Nursing Assistant, Engagement Specialist)

* Minimum 2 years' experience in a health care setting and understanding of the person-centered care philosophy strongly encourage.

* Healthcare background Required

* Customer service experience, or related experience, preferred.

* Ability to unite with people and comprehend the challenges they face Required

* Experience assisting patients with Social Determinants of Health (SDoH) needs Required

* Ability to work effectively in an interdisciplinary approach to patient care and follow a care plan.

* Experience working with patients with chronic and behavioral health needs

* Ability to know when to escalate and potentially act professionally in an emergency.

* Comfort with change and ambiguity; you understand that rapid changes to the business, strategy, organization, and priorities will come with rapid evolution of our business.

* Excellent clerical, organizational and computer skills including Word, Excel, and PowerPoint skills and communication skills - verbal and written.

* Ability to effectively multitask, essential.

* Will work from home office but regular travel to conduct member visits, provider visits and community-based visits as needed to ensure effective administration of the program. Ability to drive during daytime, nighttime, or inclement weather.

* CPR Certified or required in orientation at hire.

* Notary designation or intention to complete within six months of hire.

* Valid Driver's License with Safe Driving Record with successful clearance of a driving record check

* Ability to drive during daytime, nighttime, or inclement weather. This position requires travel to different work locations within the counties we serve.

* State minimum automobile insurance coverage.

* Successful clearance of a criminal background check
