POSITION SUMMARY:
The Community Health Worker is responsible for providing clinic-based health education and outreach services to community clients, addressing chronic disease and prevention techniques. The Community Health Worker reports to the Clinic Manager at The Community Health Worker is responsible for providing clinic-based health education and outreach services to community clients, addressing chronic disease and prevention techniques. The Community Health Worker reports to the Clinic Manager.
SPECIFIC DUTIES:
* Promote health benefit programs and clinical services in Yolo County
* Organize community meetings and focus groups
* Develop and complete a community survey and needs assessment to determine additional services, eligibility for health programs, feasibility for expanded hours, and options for another facility in the community
* Outreach to community residents to promote benefits for the uninsured
* Provide clinic-based health education services for clients and conduct community-based education and outreach addressing chronic diseases and prevention techniques
* Travel to different sites as assigned
* Attend and participate in staff meetings and training as required by supervising staff
* Maintain records of community outreach, education and meetings and complete data entry as needed for progress reports
* Prepare reports, collect data, evaluate projects, and submit appropriate information to supervisor in a timely manner
* Perform other tasks as assigned
Qualifications
MINIMUM REQUIREMENTS:
* BA degree and two (2) years of experience working in health education, or
* Five (5) years of experience working in a related field
* Public speaking ability
* Ability to communicate effectively in English written and verbal, commensurate with college level
* Valid California driver's license, proof of auto insurance, and personal transportation

* Knowledge of community resources and referral agencies
* Knowledge of computer systems and applications
* Knowledge of grammar, spelling and punctuation to compile and type reports
* Knowledge of instructional skills and educational techniques to influence and guide participants
* Knowledge of group dynamics
* Skill in interviewing and communicating to elicit information and cooperation from patients
* Skill in establishing and maintaining effective working relationships with patients, staff, and the public
* Skill in public speaking
* Skill in facilitating support groups
* Ability to communicate information on sensitive issues including sexual activities and drug related practices
* Ability to identify problems, provide information, and recommend solutions
* Ability to prepare routine records and reports using terminology common to outreach programs
* Ability to interpret, adapt, and apply appropriate written guidelines and work practices
