The selected candidate will work in our Division of Family and Clinic Services located in our Boise office. Typical Duties: + Function as a liaison between health and social service systems by communicating with identified populations with an emphasis on meeting client-centered goals and improving quality of care by aiding communication between the provider and patient; + Promote wellness and health improvements by providing culturally appropriate health information and education; + Assist clients in obtaining preventive services by referring and linking clients to appropriate care providers and/or conducting health screenings to identify need(s); + Aid in navigating health and human services systems by providing informal social support in a community setting or home to assure clients access to services they need; + Provide direct service to individuals in the form of home visiting or basic health screenings or medical follow-up (i.e. blood pressure check, blood sugar check, etc.) depending on client need; + Educate clients on maintaining wellness and/or managing chronic conditions (i.e. diabetes, asthma, vascular disease, etc.), and prioritize need(s) with individuals; + Assist with data collection to help identify community need(s); + Manage referrals in designated time frames; + Document referral activity in Electronic Medical Record (EMR) and notify provider(s) of results; + Support clinic functions which may include client scheduling, call backs and phone triaging; + To better serve our communities, assist under/uninsured clients in applying for Expanded Medicaid. Minimum Qualifications: + Some knowledge public healthcare practices; + Experience providing care, treatment, and/or recommending services to varying cultural/socioeconomic backgrounds based on client needs. Desirable Qualifications (these are not required, however, having these may increase your ranking): + Fluency in speaking, writing and reading English and Spanish; + Experience working with Hispanic/LatinX communities; + Possession of a valid driver's license.
