Community Health Worker- ObstetricsAre you someone that can provide case management with concrete follow-ups?* Are you interested in benefits effective the first day of employment?* Are you interested in providing education and care management for a specified population of patients in need of intensive care management?* Do you want to facilitate and improve access to ongoing primary health care for members of One Care program?* Do you want to provide comprehensive health care of the highest quality to everyone in our community, regardless of ability to pay.In this role you will:* Provide general administrative and clinical office support to One Care clinical staff.* Fax appropriate documents to vendors, providers, and Commonwealth Care Alliance office.* Answer incoming calls and take messages as appropriate, keep Clinicians updated on these calls and their outcomes.* Be responsible for ongoing monitoring, follow-up, and discharge planning, as appropriate.* Facilitate the updating of reporting and tracking systems with clinical and administrative One Care staff.* File appropriate documentation in the member's CCA medical record (both hard copy and electronic) according to policy and procedure.* Track, schedule, and report on annual comprehensive physical assessments for members.* Assist members with transportation to appointments as needed.The successful applicant will offer:* Two or more years experience performing a variety of administrative support functions.* Experience with contact and outreach to a variety of patients with various healthcare needs.* Ability to prioritize workload and manage multiple projects simultaneously and problem solve with minimum supervision is required.* Proficiency in word processing, Excel, Access and other computer office software knowledge strongly preferred.* Well-organized and able to communicate effectively with individuals and groups.* Bilingual in Spanish strongly preferred.* History of good attendance and positive work attitude.Please apply now and grow with LCHCThank you!
