Job Type Part-timeDescriptionEvening hours Monday, Wednesday, and Saturday. Benefits include paid time off.Essential Duties:Help clients understand their health condition(s) and develop strategies to improve their health and wellbeingFacilitate communication and client empowerment in interactions with healthcare/social service systemsServe as a liaison between health/social services and the community to facilitate access to quality services and health resourcesAssist clients with insurance applications or other paperwork and provide informal counselling, support, and follow up with them as neededAssist clients to build understanding and social capital to support healthier behaviors and lifestyle choicesWork with healthcare and social service systems to help them be culturally relevant and responsive to their service populationProvide health service screenings such as blood pressure monitoring, coaching diabetes monitoring, observe medication consumption, and medication countsCollect data and report findings to healthcare providersOther Duties:Any other duties required by supervisorEvaluate emergency situations and handles them when the Supervisor is not in the areaMaintains confidentiality of all data, including guests, employee, and operational dataEducation/Qualifications:High School Diploma/GED - CHW Certification preferred with one or more years of experienceMust agree with and sign LifePath's Statement of Christian faithMust have excellent and demonstrated interpersonal relations and both written and oral communication skillsProficiency with MS Office Suite, computer input, and database managementOther Factors to Consider:Ability to walk, stand and/or sit for long periods of time, reach, lift, bend, kneel, stoop, climb stairs, push, and pull items weighing 25 pounds or less; manual dexterity; auditory and visual skills; ability to follow written and oral instructions and procedures.Excellent reading and writing skills; typing/computer usage; proficient interpersonal relations and communicative skills; ability to follow written and oral instructions and procedures;Possess a valid Pennsylvania Driver's LicenseWorking Conditions:Inside and outside environment Salary Description $18/hr
