It provides medical and mental health services through its 11 acute care hospitals, four skilled nursing facilities, six large diagnostic and treatment centers and more than 80 community based clinics.NYC Health + Hospitals played a pivotal role during the COVID-19 pandemic, not just in treating New Yorkers but also in testing residents, breaking chains of COVID-19 transmission in a timely fashion through contact tracing, and ensuring New Yorkers can isolate and quarantine safely. NYC Health + Hospitals will play a leading role in supporting New Yorkers in the recovery from the COVID-19 pandemic, including by dedicating resources to address the disproportionate toll that the pandemic took on low-income residents and in communities of color. Though the creation of the NYC Public Health Corps, NYC Health + Hospitals will build a workforce of Community Health Workers that is focused on assisting patients with meeting their goals around health and well-being and that can be activated in case of future public health emergencies.Summary Of Duties And ResponsibilitiesThe CHW Program Manager (PM) will provide critical oversight for CHW Supervisors and CHWs at their designated Gotham sites. Within each site, one or more CHWs will provide care to patients to improve their health, strengthen their connection to primary care, and achieve patient-driven goals. The PM will serve as the primary point of contact for CHW Supervisors and CHWs throughout Gotham. The PM will meet regularly with each CHW team at their assigned Gotham sites to review program progress, provide coaching and mentoring, and identify issues for escalation to the Gotham and Central Office program leadership. The PM will also develop and strengthen relationships with community-based partners to improve referral processes for patients in need of community services. The PM will report directly to Gotham Director of Social Work and liaise with the Central Office Program Team responsible for Program Oversight.General Tasks And Responsibilities Will Include
- Provide guidance to teams of CHWs and CHW Supervisors at multiple Gotham sites across multiple disciplines which may include Adult Primary Care, Pediatrics Primary Care, patients experiencing homelessness, and/or recently incarcerated patients
- Meet regularly with assigned site-based Gotham CHWs, CHW Supervisors, and clinical leads to review key program metrics including patient enrollment targets and achievement of patient goals, training needs, and best practices.
- Conduct weekly 1:1 supervision meetings with each CHW to review patient caseload and provide feedback and recommendations on intervention approach, in the absence of a CHW supervisor or when needed.
- Meet regularly with the Central Office PHC Program team to share program progress, discuss training and coaching needs, determine opportunities for improvements to the program model, and share challenges and best practices.
- Establish strong relationships with the clinical, patient support, and administrative teams at the assigned Gotham sites to ensure all CHWs and Supervisors are able to access needed patient supports
- Present program model, goals, design, and metrics to Gotham leadership and staff to promote program buy-in; field requests/questions from Gotham staff and respond or delegate to CHWs and CHW Supervisors as appropriate.
- Escalate clinical and administrative challenges identified by the CHWs and CHW Supervisors to clinical and Gotham leadership as appropriate; work collaboratively to identify and implement solutions.
- Develop channels to facilitate communication between CHWs and clinic and hospital care teams; educate clinical teams on CHW program
- Manage patient outreach list and track enrollment progress using the electronic health record (Epic) and other programs, as needed
- Identify and carry out CHW program quality improvement initiatives such as direct CHW observation and patient follow-up interviews
- Serve as a liaison between Gotham leadership and the Central Office program team, escalating significant or widespread challenges and issues as needed.
- Liaise with key community partners to ensure available community resources and processes to access are clearly defined and disseminated to CHW Supervisors and CHWs.
- Identify, initiate, manage, and foster trusted relationships with community organizations and local stakeholders to integrate PHC activities with the local community.
- Create focused community partnerships in line with PHC goals and the social, cultural, and linguistic needs of the patient population.
- Continuously expand knowledge and understanding of community resources, services, and programs by attending community meetings, health fairs, and community advisory committee meetings to understand community issues or build relationships with community members.
- Provide technical assistance to community partners to implement referral workflows and technology (e.g. social service referral platform use).
- Collaborate with the Data Analyst and other members of the Central Office program team to develop reports and presentations with the goal of communicating program progress to internal/external stakeholders.
- Collaborate with Assistant Director of Training and other members of the Central Office program team to identify and respond to training needs; track and report on training participation.
- Represent the Program at meetings, events, and functions as needed.
Minimum Qualifications
- A Baccalaureate degree from an accredited college or university with a major in Social Science, Social Work, Public Health or related discipline and three (3) years of full-time, progressively responsible experience in the field of community relations, community organization, public health and/or social services in a public or private social or health agency; or
- A satisfactory equivalent combination of training, education and/or experience. However, all candidates must have minimum of a four-year high school diploma or its educational equivalent approved by a State's Department of Education or a recognized accrediting agency.
