Sentara Health is hiring for a Community Health Worker in Virginia Beach, VA!Status: Part Timeposition (20 hours) with a contract for 3 years.Standard working hours: Dayshift, 8am to 5pm EST; With weekend or evening hours, as neededLocation: With remote work opportunity. Will travel to the churches doctor's offices in Hampton Roads (Norfolk, VB, Newport News) for clinical studies (Blood Pressure Check and Cancer Screening) as needed. Reports to Sentara Health Research Center at Centre Pointe in Virginia Beach, VA, as needed. Job responsibilities: - Works in collaboration with the research Manager and other research team members to provide comprehensive and coordinated care.
- Engages in interventions to improve self-management skills and improve quality of life for patients needing workup for potential cancer diagnosis.
- Provides education and behavior modification strategies and provides social support to patients and families.
- Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
Required qualifications: - Driver's License - Certification - Other/National REQUIRED
- HS - High School Grad or Equivalent REQUIRED
Preferred qualifications: - Experience working in a community-based setting preferred.
- Knowledge of medical terminology preferred
