Job DescriptionSun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Duchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is seeking a full-time Community Liaison to support our Inwood Health site located in the Bronx, NY site.The Article 31 mental health clinic is a clinic identified by DOHMH as a high need area. As such, it has been selected to participate in the CONNECT Demonstration Project to provide rapid access to services and a more flexible and holistic approach to mental health that is more engaged with the community and the whole person outside the clinic walls and beyond the traditional doctor/therapist-patient interaction. The overarching goals of CONNECT and the Community Liaison is to improve the quality of life of the people served as demonstrated by progress towards recovery goals and informed by process metrics related to improved access, engagement in clinical and non-clinical services (e.g. housing, food security, successful re-entry, etc.) and reduced mental health emergency, inpatient and mobile crisis utilizationJob Competencies: Help to define main community players, including but not limited to CBOs, schools, faith-based organizations, individual citizens Design a strategy to engage community members in a meaningful way.
