Harlem Hospital Center has been a bedrock of the Harlem community since 1887. The hospital provides a wide range of medical, surgical, diagnostic, therapeutic and family support services to the residents of Central Harlem, West Harlem, Washington Heights and Inwood. Harlem is the largest hospital in the area, capable of treating the most seriously ill. Harlem provides over 90 specialized ambulatory care services, dentistry and oral surgery, behavioral health services, and community substance abuse treatment. Duties & Responsibilities Under the direct supervision of the director of social work or designee, will assist the unit social worker with providing patients with services to facilitate safe discharges. SUMMARY OF DUTIES AND RESPONSIBILITIES:
  • Promotes a positive and pleasant attitude conducive to the workplace which enhances the Mission, Vision and Value Statement of NYC Health + Hospitals Harlem Hospital.
  • Make collateral contacts
  • Send referral packets to community agencies and follow up.
  • Schedule family meetings and case conferences involving community agencies
  • Obtain patient insurance information
  • Assist patients to apply for benefits
  • Assist patients with other social needs such as food insecurity, housing issues, legal needs and transportation, filling out housing applications, shelter referrals, accompanying patents to attend housing interviews,
  • When needed orders durable medical equipment (DME).
  • Arranges transportation for discharged patients and also for patient transfers.
  • Obtains authorizations from insurance companies for next level of care: Nursing home, rehabilitation placement and/or home care.
  • Reinstatement of prior home attendant services.
  • Obtains clothing for patients from Social Work Department.
  • Contacts discharged patients to ensure services are being provided.
  • Schedule after care appointments and provide discharge summary to after care providers
  • Document each patient encounter in the electronic medical records
  • Assist colleagues as needed.
  • Accompany discharged patients to discharge location upon request
  • Additional responsibilities as assigned.
  • Meets all compliance standards/requirements such as Annual Physical Evaluation and PeopleSoft ELM Trainings and enhances personal and professional development through external educational offerings
Minimum Qualifications For Appointment to Level I
  • One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services; or
  • Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described in 1 above.
  • For Appointment to Level II In addition to meeting the qualification requirements of Assignment Level I, appointment to Level II requires:
  • Two additional years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or
  • Education and/or experience equivalent to 1 above. Study at an accredited college in sociology, psychology or other behavioral science may be substituted on the basis of 30 semester credits for each year of the experience described above. However, all persons must have at least one year of the full-time experience described above.
  • For Appointment to Level III In addition to meeting the qualification requirements of Assignment Level II, appointment to Level III requires:
  • One additional year of full-time experience as described in 1 above.
  • For Appointment to Level IV In addition to meeting the qualification requirements for Assignment Level III, appointment to Level IV requires:
  • One additional year of full-time experience.
