Every employee takes a person-centered approach that exemplifies the ICARE values (Integrity, Compassion, Accountability, Respect, and Excellence) through empathic communication and partnerships between all persons.Check us out online: www.nychealthandhospitals.orgcareers.nychhc.orgDocs4nyc.orgWork Shifts7:00 A.M - 3:30 P.M8:00 A.M - 4:00 P.M8:30 A.M - 4:30 P.M9:00 A.M - 5:00 P.M10:00 A.M - 6:00 P.MJob DescriptionHIV Services/Virology at Jacobi Medical Center has been awarded funding for the Care Coordination Program by the NY City Department of Health to provide enhanced care coordination services.Services are being delivered to patients to ensure the development of self-management skills in the areas of medical adherence, appointment compliance and overall self-care to increase viral suppression and decrease mortality and morbidity associated with HIV and other chronic conditions (e.g., Hepatitis C).The role of CCR Patient Navigators includes the provision of health education, and Directly Observed Therapy (DOT) as well as more comprehensive medical case management services such as assistance with and proactive linkage to benefits, housing, legal assistance and other Social Determinants of Health. Under the supervision of the Care Coordination Program Leadership, Including the Program Director and Coordinating Manager: Provide core program services in accordance with the client's service plan and the target service projections mandated by the funding agency including assistance with benefits, linkage with services providers, engagement in care, Directly Observed Therapy (DOT), health education, and treatment and adherence counseling.Provide community-based services such as home visits and escorts to medical, mental health and substance treatment appointments, and accompaniment to access benefits and other entitlements.Participate in case conferences and other collaborative efforts to ensure that clients are connected and retained in care.Participate in case-finding activities to outreach to clients identified as at high risk for falling out of care, showing high viral loads, and patterns of missed appointments.Perform Self-Management Assessments and other service deliverables as required with clients according to the mandated schedule.Minimum Qualifications1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,2.
