DescriptionThe role of this position(s) is to:Chore: provide the above day to day household maintenance tasks that are needed to maintain a consumer's home in a clean, sanitary and safe manner. This service includes heavy household chores such as washing floors, windows and walls, securing loose rugs or tiles, moving furniture to provide safe access and egress.Homemaker: provide housekeeping, shopping, laundry, essential errands, basic meal preparation and meal planning (not for special diets), maintain a safe environment, observing and monitoring and reporting on consumer condition and teaching those tasks.Companion: provide non-medical care, supervision and socialization to a consumer. Assist and/or supervise a consumer with light household tasks such as meal preparation, laundry and shopping. Provide services designed to develop, maintain and/or maximize the participant's independent living skills, to achieve objective of improved health and welfare to support the ability of the participant to establish and maintain a residence in the community.Individual Support and Community Habilitation: Provides services which may include locating appropriate housing, the acquisition, retention or improvement of skills related to personal finance, health, shopping, and use of community resources, community safety, and other social and adaptive skills to live in the community. Individual support and community habilitation provide supports necessary for the individual to establish, live in and maintain a household of their choosing in the community. It may include training and education in self-determination and self-advocacy to enable the individual to acquire skills to exercise control and responsibility over the services and supports they receive, and to become more independent, integrated, and productive in their communities.Personal Care Worker: This position is to provide a range of personal care assistance that is appropriate and necessary for the participant's health and well-being. To enable participant to accomplish fundamental activities of daily living including but not limited to eating, toileting, dressing, bathing, transferring and ambulation.QUALIFICATIONS: A High School Diploma/GED or life/work experience with providing care to individuals with disabilities or the elderly. Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e. nursing). Valid US driver's license and ready access to a vehicle.TRAINING REQUIREMENTS: Must maintain all certification needed in the program including: CPR, First Aid, and BFAIR Human Rights Training. May need to attend workshops, seminars or BFAIR functions as needed for job development.
