As our Community Outreach Coordinator, you'll be at the forefront of this mission serving as coordinator for the daily functions of behavioral health mobile unit, assisting the Manager and team members to ensure seamless daily operations for remote services, to include Telehealth and in-person counseling on the mobile unit. This position is responsible for set-up and relocation, as well as first contact with community to educate potential patients on the services PanCare provides while assisting with connection to other community partners as required to support increased coordination of services among Panhandle Rural Opioid Partnership members and delivery of MAT services. Position Responsibilities:
  • Responsible for remote operation setup including preparation of Telehealth equipment.
  • Greet patients and prepare them for on-site appointments or remote connection with nurse practitioner, LCSW, or LMHC.
  • Educate potential patients on services and assist in identifying patient needs to include support system, housing, transportation, family, educational/vocational, legal, and financial, acting as liaison with community resources and agencies.
  • Research and identify community resources, refer, link, and coordinate with other service providers and community facilities as needed.
  • Collaborate with Director on grant initiatives and to provide education at consortium sites.
  • Work with the Data Coordinator to provide documentation for grant reports and deliverables.
  • Maintains a high level of ethical conduct, confidentiality, dual-relationships, and professional stature.
  • Assist to coordinate and track schedule for mobile unit.
  • Ensure that the mobile unit has adequate supplies and equipment.
  • Participates in all safety programs, which may include assignment to an emergency response team.
  • Other responsibilities as assigned.
Qualifications: High School graduate or equivalent; bachelor's degree in social or health services field (preferred). Previous experience in project coordination, case manager/counseling field or similar practicum. Knowledge and skill working with diverse populations. Good interpersonal and communication skills. Able to identify and nurture strengths and support patients' rights to make independent decisions. Effective time management skills and the ability to multi-task. Ability to travel within the service area or to other locations as needed. Current or eligible for CPR certification. Physical /Mental Demands: While performing the duties of this job, the employee is regularly required to stand, walk, use hands and fingers, handle or feel, reach with hands and arms, climb or balance, talk and hear, taste or smell. The employee frequently is required to sit and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
