Our mission-driven and member-focused company is seeking a personable and self-motivated Community Outreach Specialist.The independent and self-motivated Community Outreach Specialist is an essential part of the Reema Health care model in our New York market. This role is heavily based in cold outreach interactions and quick relationship building to help members access the care they are eligible to receive. They are focused on locating members primarily through calls and texts. The focus of the role is growth of the program by serving as the initial connection with potential members; educating them about the Reema Health programming, benefits of enrollment, and documenting the interactions in detail, then handing off the ongoing support of the member to a colleague. They possess an innate ability to identify others' motivations, build strong connections, and move people to action.At the heart of Reema's approach is meeting people where they are at, wherever that may be to serve community members in a way that is personalized, innovative, and effective. This role serves as a champion in supporting and advancing our mission using a community based approach to ensure our members are able to navigate both social and health related barriers.The Community Outreach Specialist regularly works from a phone, computer or tablet, participates in virtual meetings, and receives and makes an exceptionally high volume of calls and text messages to and from members. Responsibilities:
  • Sustain a high-volume of cold outreach to members through phone and text interactions.
  • Serve as a Reema Health champion to educate and enroll prospective members about Reema's program and benefits
  • Understand overall goals and metrics for the month and proactively work to meet them.
  • Independently organize and prioritize workload with effective time and task management skills to achieve monthly goals and outcomes.
  • Utilize a high-volume of text, phone, email interactions to reach and connect with members
  • Utilize data from Reema's application to make informed, detailed outreach strategies
  • Complete verbal and written consents and other necessary documentation for eligible members
  • Utilize the Reema App to thoroughly document all work with members in a timely fashion.
  • Maintain motivation and sense of urgency in a hybrid/remote work environment
  • Complete detailed member notes for a seamless handoff to supporting Care Team members
  • Work collaboratively with our program, product, and operations teams, providing feedback to evolve and iterate strategies for the role and outreach model
  • Work a full-time 40 hour week
Education, Experience and Skills:Required
  • 2+ years of customer service and/or sales; cold outreach preferred
  • 2+ years of experience in Healthcare, Community Outreach, Social Work, Health Education Human Services, Healthcare Sales, or other related fields. Years of related experience and subsequent expertise can be substituted for years of completed education towards a bachelor's degree
  • Strong ability to initiate conversation and create connection quickly
  • Strong ability to develop, adapt, and execute outreach plans
  • Proficiencies in critical thinking, time management, strategizing, and multitasking
  • Comfortability with making high volume outbound calls and text messages
  • Metrics-driven approach to your work; meeting goals in a timely matter
  • Knowledge of social issues and community resource navigation
  • Proficiencies in critical thinking, time management, strategizing, and multitasking
  • Strong propensity towards building trusting relationships quickly
  • Excitement for working in an innovative startup environment where change is frequent
  • Basic knowledge of all related computer and software applications such as Microsoft Word, Outlook and Excel, and Video Conferencing required; G-Suite preferred
  • Technology proficient
Qualifications & Position Requirements:
  • 18 years of age or older
  • Offers are contingent upon passing a pre-employment drug screening and background check
  • Comply with federal, state and local regulations regarding patient confidentiality HIPAA
  • Use technology-based tools and systems daily
