Immediately Hiring: Community Response Community Health WorkerPeer recovery specialist (Full Time)Job DescriptionAs a Community Response Community Health Worker, you will play a crucial role in fostering community relations through outreach activities and prevention education. You will identify potential recruitment locations, engage with community members, distribute health-related resources, and ensure compliance with public health regulations. You will also provide substance use and sexual risk assessments, connect participants with necessary medical care and mental health services, and prepare them for mental health and addiction crises. Effective communication, mentorship, and accurate record-keeping are essential aspects of this role.Responsibilities+ Conduct outreach activities and prevention education to foster community relations.+ Identify potential recruitment locations and engage with community members to optimize service locations and times.+ Distribute health-related resources and tools, ensuring compliance with public health regulations.+ Provide substance use and sexual risk assessments.+ Connect participants with medical care, substance use, and mental health treatment, and other services.+ Solicit and track participant feedback to ensure effective referrals and respectful treatment.+ Prepare participants for mental health and addiction crises, and develop communication skills.+ Maintain communication between community members and providers.+ Model participant engagement and provide guidance to staff, peers, and volunteers.+ Maintain accurate records and data on program activities and participants, and provide timely reports.+ Prepare and maintain program sites and materials, and manage inventory.+ Complete daily vehicle maintenance logs and ensure vehicles are clean and ready for service.+ Be available for varied schedules, including evenings and weekends.+ Perform other duties as assigned.Essential Skills+ Associate's degree with an approved combination of experience, certification, and/or education.+ Minimum of three years of experience in client case management.+ Active Community Health Worker (CHW) certification preferred.+ Valid driver's license.+ Positive perspective about law enforcement and working with law enforcement.+ Ability to work in inclement weather.+ Willingness to work holidays if scheduled, with the option to receive a day off if a holiday is worked.Additional Skills & Qualifications+ CHW certification required or ability to obtain within 90 days.Work EnvironmentThis position is part of the community emergency response-HEARTeam (Hartford Emergency Action Response Team). The peak shift is from Tuesday through Saturday, 1 PM to 10 PM. You will be expected to work in various weather conditions and may need to work holidays if they fall on your scheduled shifts. If a holiday is worked, you will be able to receive a day off.Pay and BenefitsThe pay range for this position is $22.00 - $24.00Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
