SCLHSA covers 7 parishes (Assumption, Lafourche, St. Mary, St. John, St. James, St. Charles, and Terrebonne parishes). The Community Service Professional must be flexible and familiar with a myriad of supports and services through the agency as this staff person may be placed within different units within the office and/or perform a percentage of work from several units at one time. The incumbent is required to have advanced knowledge of promulgation and policies and procedures in multiple statewide program elements including the Development Disabilities Act. The incumbent must also be skilled in interviewing and assessment techniques to determine criteria for assessing development disability service. He/She must also be able to establish good rapport and working relationships with individuals served by the agency, their families, professional service agencies and other to assist families/individuals in funding requests/decisions and in understanding the funding restrictions/limitations of the agency. The incumbent is required to possess organizational skills that will enhance the smooth operation of office, the work assigned and programs for which he/she is responsible. Statewide program elements that the incumbent may be assigned included: Single Point of Entry Review of application request, Family Support, Flexible Family Funds, Court/ custody case/ Family In Need of Services, Interagency Service Coordination, School Transition meetings, Out of home living exploration, Nursing Home admission approval, ICF/DD admission approval, Crisis Planning and management, Emergency Waiver applications, and Support Coordination. Candidates with 120 or more semester hours of credit, but without a degree, must also have at least two years of full-time work experience to substitute for the baccalaureate degree.A master's degree in psychology; counseling; social work; sociology; criminal justice; nursing; public health; public health administration; public administration; hospital administration; education (with eighteen semester hours in psychology, special education or early childhood education); speech communication/pathology; physical therapy; occupational therapy; therapeutic recreation; music therapy; or family and consumer sciences (with a concentration in child, family and social services) will substitute for a maximum of one year of the required experience.A Ph.D. in the above fields will substitute for all of the required experience.NOTE:Some positions may require regulation as a Registered Social Worker (RSW), Licensed Master's Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).NOTE:Any college hours or degree must be from an accredited college or university.Job ConceptsFUNCTION OF WORK:To provide experienced level implementation, coordination, monitoring and evaluation of programs within offices specializing in services for those with developmental disabilities.LEVEL OF WORK:Experienced.SUPERVISION RECEIVED:General from a Community Services Supervisor, a higher-level professional or an administrator.SUPERVISION EXERCISED:NoneLOCATION OF WORK:Department of Health and Hospitals (DHH) and Human Services Districts/Authorities statewide.JOB DISTINCTIONS:Differs from the Community Services Professional 1 by the presence of independent performance of duties under general supervision.Differs from the Community Services Professional 3 by the supervision received and presence of less independent and complex assignments.Examples of Work90% Essential Functions:+ Receive/screen inquires/calls to persons contacting SCLHSA Developmental Disabilities requesting assistance; determine the need for support services by initiating appropriate disposition for assistance to the person.+ Conduct complex assessments utilizing assessment tools such as ICAP and DD SNAPC.+ Conduct interviews to determine initial and continued eligibility for persons who request services in the Developmental Disabilities Service system and Flexible Family Funds.+ Develop support plans/support profiles for persons seeking system entry or services within the Developmental Disabilities Service.+ Conduct detailed review of medical records, psychological evaluations, educational reports, and other available documentation to determine if information supports eligibility requirements as established by state law for Developmental Disabilities Services and Flexible Family Funds.+ Prepare documentation for review by Entry Review Team which consists of credential professionals to determine Entry eligibility decisions that require a second level review.+ Determine initial and continued eligibility for children who have been assigned slots for Flexible Family Funds program and ensure ongoing monitoring of eligibility for children receiving Flexible Family Funds benefits maintain their eligibility on a quarterly basis.+ Analyze evaluative information, identify and prioritize support needs, resources, develop components of support plan and/or initial referrals for supports from alternative entities.+ Assist individuals in developing service plans through identifying appropriate goals/services and make recommendations for specific support needs based on information gathered through assessments, personal interviews and information obtained from outside sources.+ Develop and implement Support Plans outlining needs and funding requests for presentation to the family support review committee in anticipation of funding decisions.+ Outline appropriate specialized services for program participants based on assessments using the ICAP and DD SNAP assessment tool and other pertinent data gathered during interview with program applicant/participant.+ Prepare appropriate documents for funding of approved agency services which includes a thorough explanation to the individual and/or responsible party an explanation regarding fiscal process for reimbursement.+ Plan and coordinate services for program that is appropriate to the need of the individual.+ Monitor program as required by program policy to ensure individual satisfaction and fiscal soundness.+ Create and maintain a resource list of community resources, advocacy organizations and provider agencies based within SCLHSA area that are available for referrals for supports for individuals. Continue to become knowledgeable of community resources available for referrals for supports for individual needs.+ Maintain knowledge of Developmental Disabilities state and federally funded resources.+ Coordinates the exploration of alternative living arrangements for clients/families requesting such as identified in plan of care.+ Participate in Interagency Coordination meetings and Family in Needs of Services meetings in order to provide information on services offered through Developmental Disabilities Service system.+ Participate in school transition meetings and workshops within the school system representing the agency and explain the criteria for participation in the Developmental Disabilities Service system.+ Attend court hearings and explain the criteria for Developmental Disabilities services to court personnel and the policy on funding community services.+ Respond to crisis situations as necessary to define the situation with available agency resources or appropriate referral to outside agencies. Respond to Adult Protective Services, Elderly Protection Services referrals within two days of request for collaboration and accompanying APS and EPS staff as necessary with assistance in reducing risk of individuals in need.+ Prepare reports; maintain case records; complete professional correspondences relevant to all statewide programs assigned.+ Negotiates terms for complex social service contracts through direct personal contact with administrative provider agency personnel.+ Conduct on site interviews with provider agency administrators and assure program compliance with contract expectations.+ Prepare appeals packets for supervisor review and approval for program identified for submission to the Division of Administrative Law and participate in appeals before Administrative Law Judge by presenting evidence for agency decision during hearing.+ Serves as liaison and coordinator between individuals, their families, local and state agencies, the community, and service providers.+ Plan and coordinate a portion of a statewide program element in a specific service area.+ Provide training to less experienced staff in selective areas of expertise or programmatic areas identified by supervisor or administrator.+ Conduct in-service or educational presentations for community groups, agencies, providers, etc. on developmental disability eligibility and programs.10% Other Duties:+ Participate in staff and unit meetings.+ Participate in individual meetings with supervisor to keep him/her abreast of caseload situations and review PES as necessary.+ Participate in inter-unit collaborations for successful coordination of services of individuals in the Developmental Disabilities Service system.+ Participate in professional and educational trainings to maintain and improve professional skills.+ Participate in emergency preparedness training and activities to be prepared in the case of such situations.+ Participate in agency internal committees as requested by supervisor or administrator to assist SCLHSA DD in review on cases for eligibility and/or funding decisions.+ Represent agency as requested by supervisory personnel by being prepared to explain agency programs to the general public and/or families/groups/service providers.+ Participate and complete other duties as necessary for the office overall successful functioning as assigned by supervisor or administrator.Louisiana State Government represents a wide variety of career options and offers an outstanding opportunity tomake a differencethrough public service.
