Community Support SpecialistJob DetailsJob LocationCSS DDMI ESH Mercer - Mercer County, NJPosition TypeFull TimeEducation Level4 Year DegreeSalary Range$18.00 - $20.00 HourlyJob ShiftOvernight ShiftThe duties of the Community Support Specialist include contributingto the development, implementation, monitoring, and updating of consumers' individual habilitation plans; therapeutic rehabilitative skill development; implementation of Illness Management and Recovery (IMR) principles and interventions; coordinating and managing services, both mental health and physical health; crisis intervention, as needed. Provision/coordination of services aimed at active rehabilitation and support services to achieve restoration of functioning and the achievement of community integration; maintaining own appointment schedule and meeting work productivity requirements; other duties as assigned by management staff.Community Support Services offers a variety of supports to help Burlington County adults with a serious mental illness remain living successfully in the community. Services include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills, education/employment assistance, transportation and 24-hour on-call support.Enhanced Community Support Services is also available for adults being released from state psychiatric hospitals.
